Job Description - LPN Consultant

Description

- Review and sign off on individualized care plans and assessments for residents.

- Authorize and manage medication administration in accordance with state regulations and facility policies.

- Collaborate with healthcare staff to ensure comprehensive care for residents.

- Conduct regular evaluations of resident health and well-being.

- Maintain accurate and up-to-date documentation of care plans and medication records.

- Participate in staff training and development programs as needed.

Requirements

  • Current LPN license in the state of Indiana.
  • Previous experience in assisted living or memory care settings preferred.
  • Strong knowledge of medication management and care planning.
  • Excellent communication and interpersonal skills.
  • Strong organizational skills and attention to detail.
  • Compassionate and patient-centered approach to care.
