Job Description - LPN Crisis Care

Trustbridge Hospice, a part of Empath Health, is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide Continuous Care to patients in the Broward County area on the following schedule: 7pm - 7:30am and every other weekend. What you'll Do

As a Continuous Care LPN, you will play a vital role in supporting patients and families during some of the most critical moments in care.

Deliver compassionate, high-quality care aligned with Trustbridge Hospice’s mission, vision, and values

Work under the direction of the Registered Nurse and primary physician to assess patient and family needs

Coordinate and provide individualized care plans that prioritize comfort and dignity

Utilize monitoring, intervention, and teaching skills to enhance quality of life

Provide emotional support and reassurance to patients and families during overnight care

Ensure exceptional, personalized service and a positive experience for every patient and family

Maintain a strong commitment to patient satisfaction and overall care excellence

What You'll Need

Current license to practice as a Licensed Practical Nurse in the State of Florida.

Current Basic Life Support Certification.

Successful completion of 30 hour IV Course meeting State of Florida Board of Nursing requirements preferred.

Hospice Certification preferred.

Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
