Key Responsibilities:

• Direct Care: Provide direct care and assist with care as directed, consistent with your scope of practice and competency.
• Collaboration: Initiate the SBAR process and collaborate with the RN for assessments and recommendations. Participate in rounds with physicians and other team members.
• Resident Evaluations: Engage in resident evaluations and care planning activities, revising assessments and care plans as needed.
• Medication Management: Oversee Medication Aide/Tech in medication/treatment administration, administer treatments, and manage medication safety and effectiveness.
• Documentation: Document care performed and observations of resident status promptly, adhering to clinical documentation guidelines.
• Emergency Response: Assist with emergencies and administer cardiopulmonary resuscitation when necessary.
• Clinical Admission: Complete clinical admission paperwork for new admissions and assist with the clinical discharge process.
• Consultation: Assist consultants during services, follow up on recommendations, and implement pharmacy review/drug regime review recommendations.

Requirements

Education

Graduate of accredited LPN School of Nursing.

Licenses/Certification  

Current valid LPN license in the state employed.

Valid CPR certification.

Experience

Six months experience in a long-term care environment preferred.
