This is a full-time position, Monday - Friday, 8:30am to 5:00pm. Rotating on call weekends and holidays.

Wage **$25.97 and up** based on experience.

As an LPN Desk Nurse, you will be a leader in completing work that involves clerical duties to maintain an organized unit and allow nursing staff to focus on patient care. You will also maintain a line of communication between the patient and clinical staff while reporting directly to the RN Case Manager and Director of Nursing.

Responsibilities include but are not limited to:

• Be a central communicator and resource for families, providers, and other departments
• Meet routinely with DON and Case Management team to discuss personnel and patient care status, issues, and information
• Assess daily patient care needs and develop and distribute patient care assignments appropriately
• Supervise all documentation of patient information
  
• Prepare records for patients
• Provide day-to-day guidance, support, and direction to direct patient care staff
  
• Participate in patient care plan meetings
  
• Train and orient staff as necessary
• Assist nursing management team with staff meetings as needed to keep patient care staff informed of changes in patient care needs or operations to improve delivery of care
  
• Assume primary responsibility in an emergency situation
  
• Collaborate with direct patient care team in making decisions to benefit patient care
  
• Participate in the completion of short and long-term care plans
  
• Admit new patients according to facility procedure
• Assist in ensuring the educational needs of patients are met and educate the patient and family
• Supervise the safe and effective use of all equipment involved in direct patient care
• Promote efficient use of medical supplies
• Attend and participate in Quality meetings, as requested
• The ideal candidate will be an exceptional communicator, detail-oriented, and be able to work in a fast-paced environment
