Job Summary
We are seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing care to individuals receiving private home health services under DODD guidelines. The ideal candidate will have a passion for working with individuals with developmental disabilities, strong clinical skills, and the ability to work independently while adhering to care plans and regulatory requirements.
Key Responsibilities
- Provide direct nursing care based on the individual's Plan of Care (POC) and physician orders.
- Administer medications, treatments, and therapies as prescribed.
- Monitor and document the individual's health status, including vital signs and any changes in condition.
- Assist with activities of daily living (ADLs) as needed.
- Educate individuals, families, and caregivers on health conditions, medication administration, and treatment plans.
- Maintain accurate and timely documentation in compliance with DODD regulations and agency policies.
- Collaborate with RNs, physicians, caregivers, and interdisciplinary teams to ensure coordinated care.
- Follow infection control and safety protocols to provide a safe environment for the individual.
- Respond to emergency situations appropriately and follow agency protocols.
Qualifications
- Current and active Licensed Practical Nurse (LPN) license in the state of Ohio.
- Experience working with individuals with developmental disabilities (preferred but not required).
- Knowledge of DODD regulations and Medicaid waiver services (preferred).
- Valid CPR/BLS certification.
- Ability to pass a background check and drug screening per state and agency requirements.
- Strong clinical assessment, critical thinking, and communication skills.
- Compassionate, patient, and dedicated to providing high-quality care.
- Ability to work independently and follow care plans.
