We are currently seeking a Licensed Practical Nurse (LPN) to serve as a Clinical Lead in Wooster, OH and surrounding areas. Responsibilities

• Conduct client assessments and maintain accurate electronic health records (EHR).
• Collaborate with healthcare team to develop and implement care plans tailored to individual client needs.
• Assist with oversight of activities of daily living and ensure patient comfort and safety.
• Adhere to HIPAA regulations and maintain confidentiality of patient information.

Requirements

• Valid Ohio Driver’s License, required
• Reliable transportation, required
• Proof of auto insurance, required
• Licensed Practical Nurse
• Must be able to pass a criminal background check and drug test
• Ability to lift, push, pull, and carry up to 50lbs
