TRUE Diabetes Neuropathy & Wound Solutions is hiring!
Here are the essentials:
- Full-time LPN to work Monday through Thursday in our diabetes center across from the hospital in Gilbert offering advanced therapies.
- Excellent venipuncture skills required, like magician level hands :). We are passionate about phenomenal patient experiences
- Operate diabetes IV therapy treatment plans: Starting peripheral IV, preparation of IV solution, monitoring patients during the 2-3 hour infusion time, performing blood glucose level checks regularly, removing IV catheter safely, and instructing patients in appropriate aftercare.
- Looking for an LPN that is passionate about making a major difference in people's lives that suffer from diabetes and its comorbidities.
Responsibilities:
- Always upholding best practice standards in all tasks.
- Assisting in the treating of diabetes, neuropathy, and chronic wounds under the supervision of our specialty Nurse Practitioner.
- Patient preparation: taking vitals, assisting patients with filling out clinical portions of their paperwork if necessary.
- Exam room preparation: ensuring all necessary equipment for the patient visit is ready, ensuring the room is appropriately cleaned/ restocked between patients,
- Assisting our Nurse Practitioner in the conducting of comprehensive examinations for wound care, diabetes, and neuropathy.
- Assisting in the educating of patients on lifestyle habits; including recommended preventative care, and disease management.
- Build strong direct relationships with patients/ team members.
- Facilitate referrals to other healthcare professionals and medical facilities. Assist in the documentation of patient care activity and communication with the patient, physician, and pharmacist.
- Train patients and/or caregivers in procedures and equipment. Provide direct patient care as required, i.e. peripheral IV starts, lab draws, dressing changes as directed by our Nurse Practitioner.
- Participate in Telemedicine when necessary.
- Monitor compliance, competence, and tolerance to prescribed therapy, interventions, and progress.
- Provide other healthcare professionals with feedback on patients; at all times evaluating quality of service and initiating changes to ensure best practice in regards to procedure/protocol.
- Maintain and update, per policy, clinical documentation which includes, but is not limited to; daily visit logs, AOB's, delivery tickets, on-call logs, assessments, subsequent notes, care plans, medication profiles, plan of treatment, case conference notes, progress notes, discharge summary and any other paperwork required by the company.
- Must maintain current and be in good standing with all state, federal and company set requirements to fulfill duties and responsibilities of the assigned position.
- Completion of narrative notes and charts within 24 hours of appointment.
Education & Experience:
- Minimum education: LPN with IV Certification.
- 2 or more years experience in the HealthCare Field.
- Sales Experience is preferred
