Aci Health is seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients in LTC environment.

Responsibilities

• Deliver direct patient care, including monitoring vital signs and administering medications as prescribed.
• Assist with IV infusion therapy and ventilator management for patients requiring respiratory support.
• Collaborate with healthcare professionals to develop and implement individualized care plans.
• Conduct thorough assessments of patients' health status and document findings accurately.
• Provide education to patients and their families regarding treatment plans, medications, and home care needs.
• Participate in discharge planning to ensure a smooth transition from inpatient care to home or other facilities.
• Maintain compliance with HIPAA regulations to protect patient confidentiality at all times.
• Engage in ongoing professional development to enhance nursing skills and knowledge.

Requirements

• Must have at least one (1) year of experience.
• Valid Licensed Vocational Nurse (LVN) license in the state of practice.
• Strong communication skills, both verbal and written, for effective patient interaction.
• Ability to work collaboratively within a multidisciplinary team environment.
