Key Responsibilities:

Patient Care:

  • Assists RNs with patient assessments, including vital signs, basic physical assessments, and data collection.
  • Administers prescribed medications safely and accurately via various routes (e.g., oral, intramuscular, subcutaneous) in accordance with hospital policy and scope of practice.
  • Performs treatments and procedures such as wound care, catheterizations, and specimen collection.
  • Monitors and observes patient conditions, recognizing and reporting changes or adverse reactions to the supervising RN promptly.
  • Assists patients with activities of daily living (ADLs) including hygiene, mobility, and feeding.
  • Provides basic patient education under the direction of an RN.

Documentation:

  • Accurately and timely documents all patient care activities, observations, and medication administration in the electronic health record (EHR) or other designated system.
  • Maintains confidentiality of patient information in accordance with HIPAA regulations.

Communication & Collaboration:

  • Communicates effectively with patients, families, and members of the healthcare team.
  • Collaborates with RNs, physicians, and other healthcare professionals to develop and implement individualized patient care plans.
  • Participates in shift reports and hand-offs to ensure continuity of care.

Safety & Quality:

  • Adheres to all hospital policies, procedures, and safety standards, including infection control protocols.
  • Ensures a clean, safe, and organized patient environment.
  • Participates in quality improvement initiatives as directed.

Professional Development:

  • Maintains current LPN licensure in good standing.
  • Completes all mandatory in-service education and continuing education requirements.
  • Participates in ongoing professional development to enhance nursing knowledge and skills.

Qualifications:

  • Education: Graduate of an accredited Licensed Practical Nurse (LPN) program.
  • Licensure: Current, unencumbered LPN license in the State of [State where hospital is located].

Experience:

  • [Optional: 1+ years of experience as an LPN in an acute care setting preferred, or New graduates encouraged to apply.]

Skills & Abilities:

  • Demonstrated knowledge of nursing principles and procedures.
  • Ability to perform basic patient assessments and interventions.
  • Proficient in medication administration.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment and manage multiple priorities.
  • Basic computer proficiency for electronic health record documentation.
  • Ability to stand, walk, bend, lift, and assist with patient transfers as needed.
  • BLS (Basic Life Support) certification required upon hire; ACLS (Advanced Cardiovascular Life Support) may be required for specific units.

Work Environment:

  • Hospital setting with exposure to various patient conditions, medical equipment, and potential infectious diseases.
  • Requires working rotating shifts, weekends, and holidays as needed.

Physical Demands:

  • Frequent standing, walking, bending, lifting, pushing, and pulling.
  • Ability to lift and/or move up to 50 pounds with assistance.
  • Visual and auditory acuity sufficient to perform duties safely and accurately.
