Job Description - LPN - Palliative Care

Job Summary



Licensed Practical Nurse (LPN) embedded within our Intake Department with dedicated support responsibilities for our Palliative Care Program. This office-based role provides direct clinical and administrative support to Partners in Home Care (PIHC) Nurse Practitioners and plays a critical role in ensuring smooth coordination of palliative care services. The LPN will manage communication, referrals, patient records, and scheduling—all with a focus on supporting patients and families living with serious illness.



Key Responsibilities



Administrative & Clinical Support to Palliative Care Team



  • Serve as the primary LPN support to palliative NPs in the office setting.

  • Respond to all incoming palliative care phone calls and inquiries with professionalism and empathy.

  • Screen all incoming palliative referrals for appropriateness and completeness.

  • Build and maintain complete and accurate patient charts, securing referral documents from various sources.

  • Track and verify cost sheet components and patient eligibility documentation.


Communication and Patient Coordination



  • Perform proactive patient and caregiver outreach: as directed by the NP



  • Tuck-in phone calls

  • Post-hospitalization follow-ups

  • Medication coordination and follow-up



  • Facilitate communication between the palliative care team and referring providers, patients, and families.

  • Process orders from Nurse Practitioners and Social Workers, routing as needed (e.g., diagnosis clarification, scheduling changes).

  • Schedule visits across palliative disciplines (NP, LCSW).

  • Fax visit notes to Primary Care Providers and maintain timely documentation.


 


Program Coordination and Quality Monitoring



  • Participate in and contribute to the daily Palliative Care needs (meetings, census review, scheduling, care planning).

  • Schedule and document care conferences as directed by NP and or LCSW.

  • Monitor palliative care census and referral channel daily.

  • Track patient movement from Home Health (HH) to Palliative (PAL) to Hospice (HSPC), facilitating warm handoffs and documentation updates.

  • Discharge palliative charts when patients transition to hospice services.

  • Maintain chart accuracy to support audit readiness and compliance with regulatory and internal standards.

  • Monitor EMR entries and documentation for accuracy to support pre-bill audits and clean billing processes.


Operational Support



  • Manage office supply needs related to Palliative Care, including PPE preparation for NPs.

  • Assist in projects related to clinic or office operations, as needed.

  • Participate in training led by the Intake Manager and ongoing collaboration with the Palliative Care team.


Other Responsibilities



  • Participates in other duties as assigned.


Knowledge Skills and Abilities



  • Communication & Time Management: Demonstrated ability to communicate effectively and professionally with patients, families, and interdisciplinary team members. Strong organizational and time management skills to prioritize multiple tasks in a dynamic office environment.

  • Clinical Review: Ability to review and decipher referrals and chart information as it relates to palliative needs. Understanding of Advance Directives, POLST and Medical – POA terminology. Will have access to EPIC, Cerner, and MT prescription drug registry.

  • Compassion & Empathy: Committed to delivering patient-centered care with compassion, dignity, and sensitivity to the physical and emotional needs of patients and families facing serious illness.

  • Collaboration & Teamwork: Strong ability to work collaboratively within a multidisciplinary care team, contributing to shared goals and ensuring continuity of care.

  • Data Entry & Documentation Accuracy: High attention to detail and accuracy in EMR data entry, supporting compliance, audit readiness, and pre-bill documentation standards.

  • Adaptability & Initiative: Comfortable working in a fast-paced, evolving environment with the ability to adapt to program needs, take initiative, and follow through on tasks with minimal oversight.


  


Minimum Requirements



Education/Certifications    



  • Licensed Practical Nurse (LPN) License: A valid LPN license in the state of Montana. 


Experience



  • Prefer Knowledge of Palliative Care Principles: Understanding the principles of palliative care, including symptom management, psychosocial support, and end-of-life care.
