In this role, you will:
Provide ongoing support relative to clinical data extraction, collection, and submission
Assist in the coordination of outreach efforts to request, collect, and secure clinical documentation from Primary Care Providers (PCP) and/or Specialist(s)
Review assigned medical records to abstract specified and required clinical data elements related to gap closure
Assist with training of staff at PCP practices and provide feedback on processes and documentation
Attend monthly meetings with insurance companies, navigate portals and maintain appropriate documentation to track performance
Other duties and responsibilities, as assigned
We require:
Current NYS LPN license required.
Minimum of three (3) years of experience in the healthcare industry, in HEDIS and/or quality improvement.
Experience in a managed care health services environment preferred
Demonstrated knowledge of data collection, medical record review, and data submission.
Ability to effectively prioritize work, multi-task, and achieve goals on time; work independently and self-motivate
Proficiency in the use of Electronic Medical Records and computer applications (Google Suite, Microsoft)
Excellent oral and written communication skills
Hours: Monday-Friday Salary up to $35 per hour based on experience and education.
