The Grand Rehabilitation & Nursing at Rome is seeking a skilled and motivated LPN Supervisor to join our nursing leadership team. The LPN Supervisor supports nursing staff, oversees unit operations, and ensures residents receive high-quality, timely care in accordance with care plans and facility standards.

This role is ideal for an experienced LPN ready to step into a leadership position and make a meaningful impact on resident care and team success.

Available Shift Options

• 
  

  Weekends Only: 7:00 PM – 7:00 AM (12-hour weekend nights)

  
  


• 
  

  Full-Time: 11:00 PM – 7:00 AM (Overnight shift)

  
  

LPN Supervisor Responsibilities

• 
  

  Supervise and support CNAs and LPNs during assigned shifts.

  
  


• 
  

  Ensure residents receive high-quality care in alignment with individualized care plans.

  
  


• 
  

  Conduct unit rounds to monitor resident status, staff performance, and overall safety.

  
  


• 
  

  Assist with medication administration, treatments, and clinical tasks as needed.

  
  


• 
  

  Respond to resident incidents, concerns, and changes in condition.

  
  


• 
  

  Collaborate with RNs, physicians, and interdisciplinary team members.

  
  


• 
  

  Review staff documentation for accuracy and regulatory compliance.

  
  


• 
  

  Reinforce infection control practices, safety guidelines, and facility policies.

  
  


• 
  

  Participate in quality assurance activities, shift reports, and staff development efforts.

  
  


• 
  

  Support new staff onboarding, mentoring, and ongoing training initiatives.

  
  

Qualifications

• 
  

  Current NYS Licensed Practical Nurse (LPN) license

  
  


• 
  

  Prior long-term care or supervisory experience preferred

  
  


• 
  

  Strong clinical, organizational, and leadership skills

  
  


• 
  

  Excellent communication and teamwork abilities

  
  


• 
  

  Ability to work independently and manage multiple responsibilities
