WorkFit Medical Staffing is actively searching for compassionate and skilled LPNs to support patients in a rehabilitation setting. This is an excellent opportunity for LPNs who are passionate about patient recovery and want to make a meaningful impact in their lives.

Key Responsibilities:

• Document patient history and enter data into electronic health records
• Administer medications, injections, and immunizations as prescribed
• Monitor and record vital signs including blood pressure, temperature, and weight
• Provide basic wound care and assist with bathing, dressing, and mobility
• Observe and report patient responses to medications and changes in mental health
• Support patients emotionally and ensure understanding of discharge instructions
• Supervise Nursing Assistive Personnel (NAP) and oversee daily care routines
• Track fluid and nutritional intake/output and assess for skin integrity issues

Qualifications:

• Current New York State LPN license
• Minimum of 1 year experience in Long-Term Care or Rehabilitation setting
• Strong clinical assessment and communication skills
• Ability to work collaboratively in a multidisciplinary team
