Physical Therapist – LTC/SNF - Travel Position

Pay: $2,055/week

Job Description:

We are seeking a compassionate Physical Therapist to join a skilled nursing and long-term care facility in Franconia, NH, on a travel contract. The Physical Therapist will assess residents, develop individualized treatment plans, and deliver therapy services designed to improve mobility, restore function, and promote independence. The ideal candidate is patient-centered, detail-oriented, and experienced in geriatric rehabilitation.

Key Responsibilities:

• Conduct patient evaluations to determine mobility, functional status, and therapy needs.


• Develop and implement individualized treatment plans in coordination with physicians and care teams.


• Provide therapeutic interventions such as gait training, strengthening, balance activities, and mobility support.


• Educate residents, families, and caregivers on therapy goals, safety, and the use of assistive devices.


• Document evaluations, progress notes, and treatment outcomes accurately in compliance with facility and regulatory standards.


• Collaborate with interdisciplinary staff to ensure coordinated, patient-centered care.


• Promote safety, independence, and improved quality of life through evidence-based therapy practices.

Qualifications:

• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy from an accredited program.


• Current and valid Physical Therapist license in New Hampshire (or compact license).


• Minimum of 1 year of recent PT experience, preferably in skilled nursing or long-term care facilities.


• Current certification in Basic Life Support.


• COVID-19 vaccine and recent tuberculosis test.


• Strong clinical, communication, and organizational skills.


• Compassionate, patient-centered approach with experience supporting geriatric populations.
