Physical Therapist LTC/SNF – Travel Position

Pay: $2,125/week

Job Description

We are seeking a compassionate Physical Therapist to join a skilled nursing and long-term care facility in Buffalo, MO, on a travel contract. The Physical Therapist will evaluate residents, design individualized treatment plans, and deliver therapy services focused on improving mobility, restoring function, and promoting independence. The ideal candidate is patient-centered, adaptable, and experienced in geriatric rehabilitation.

Key Responsibilities

• Conduct patient evaluations to assess functional abilities and therapy needs.


• Develop and implement personalized treatment plans in collaboration with physicians and interdisciplinary teams.


• Provide therapeutic interventions including strengthening, gait training, balance activities, and mobility support.


• Educate residents, families, and caregivers on therapy goals, safety strategies, and use of assistive equipment.


• Document evaluations, treatment sessions, and outcomes accurately in compliance with facility and regulatory standards.


• Work with nursing and therapy staff to deliver coordinated, patient-centered care.


• Promote independence, safety, and improved quality of life through evidence-based practices.

Qualifications

• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy from an accredited program.


• Current and valid Physical Therapist license in Missouri (or compact license).


• Minimum of 1 year of recent PT experience, preferably in skilled nursing or long-term care.


• Current certification in Basic Life Support.


• COVID-19 vaccine and recent tuberculosis test.


• Strong assessment, communication, and organizational skills.


• Compassionate approach with experience supporting geriatric populations.
