Program: Nursing
Department: Health Services
Reports to: Assistant Nurse Manager
Compensation: $33.10-$43.67 per hour, plus $1.75 per hour Shift Differential Premium for hours worked between 6pm-6am
Schedule: Monday-Friday 8:00am-5:00pm
Status: 100%, Full-Time, Non-Exempt, Union
Probation: 90 days

DEPARTMENT SUMMARY: As the nation’s largest and most experienced provider of LGBT medical and mental health services, we help LGBTQ people lead healthier, happier lives, by keeping their minds and body well. All services are free or low cost. We’re also one of the few Federally Qualified Health Centers in the nation with providers who specialize in primary care for LGBTQ people and with a research team working to advance the care and treatment of people in our community.
JOB SUMMARY: Under the supervision of the Assistant Nurse Manager, the LVN Care Coordinator will collaborate with other healthcare providers in

implementing/supporting

care plans, support clinic flow, coordinate patient care, promote continuity of care, and facilitate appropriate delivery of health services. Services will be provided to support quality patient care in compliance with local, state, and federal regulations and accreditation standards.
ESSENTIAL FUNCTIONS:
• Serve as part of a multidisciplinary care team to provide a range of patient care services.
• Work in collaboration with Medical Providers and other Health Services staff to maintain an effective and efficient program.
• Facilitate patient access to healthcare.
• Within the scope of license, provide skilled nursing care, administer medications, treatments, vital signs, and provide patient education regarding medications, diagnosis, and prevention.
• Observe patients for any medication side effects or reactions; document side effects or reactions and report to Medical Provider and Nursing Management.
• Coordinate and communicate with Medical Providers, patients, and pharmacies regarding patient’s medication prescriptions and refills.
• Coordinate and support activities related to prior authorizations and Patient Assistance Programs (PAPs) needed for PrEP and all other medications.
• Assist in care coordination related to Transgender Healthcare including gender affirming surgery navigation as well as education on hormone administration and replacement therapies.
• Maintain complete and accurate files documenting all care and services provided in the patient’s health record.
• Maintains clear and concise communication, including case presentations, written progress notes, medication lists, summaries, and reports.
• Coordinate referrals and obtain consult notes as needed per clinic protocol to all specialty consultants and ancillary health care services.
• Reduce gaps in services by promoting continuity of care and follow-up of patients.
• Serve as a liaison between Health Services and specialist consultants; and work in partnership to provide consistent, quality care to patients.
• Support coordination of care transition for patients engaged in home health, hospital discharge, and hospice care etc.
• Assist with health care education for patient, partner, and family members; provide health promotion and prevention teaching.
• Provide and reinforce education from Medical Provider pre-approved printed educational materials.
• Increase the access and utilization of medical services by eliminating barriers and promoting early and on-going intervention and adherence.
• Assist with continuous improvement of total quality care as it relates to nursing services and medication treatment and adherence.
• Monitor clinic refrigerators and maintain temperature logs; notifies supervisor of any out of range temperatures.
• Interact in a professional manner with health services staff and patients.
• Exercise appropriate judgment and decision-making skills.
• Use standard office machinery including personal computers and printers in performance of duties.
• Assist in data auditing tasks, prepare reports as required, and submit in a timely manner.
• Participate in staff meetings, agency meetings, planning meetings, and other meetings as needed.
• Comply with requirements of OSHA, HIPAA, and other applicable regulations.
• Other duties as assigned.
JOB QUALIFICATIONS AND EXPERIENCE:
• Knowledge of or experience working with the LGBTQ community and familiarity with issues of particular relevance to LGBTQ people.
• A passion for the Center’s work and its mission to make the world a better place for LGBTQ people.
• Minimum 3 years of experience in direct patient care; preferably in an ambulatory care setting.
• Current California LVN licensure required.
• Current AHA CPR certification required.
• Basic knowledge of infectious diseases, e.g., HIV/AIDS, STI’s, and state/ federal reporting requirements.
• Basic knowledge of care coordination and navigation throughout the managed care process i.e., prior authorization, specialty care coordination, patient assistance program.
• Excellent verbal and written communication skills, including strong organizational, detail, and interpersonal skills.
• Excellent Computer skills and knowledge of Microsoft Office, word processing, Electronic Health Records, and other software systems.
• Ability to be self-motivated and dedicated to initiating and completing complex tasks.
• Ability to handle multiple tasks, be detailed oriented, and maintain strict confidentiality of medical information.
• Able to work well independently with minimum supervision.
• Bilingual skills, a plus (English/Spanish fluency preferred).
• Demonstrated ability to work effectively with people of diverse races, ethnicities, nationalities, sexual orientations, gender identities, gender expression, socio-economic backgrounds, religions, ages, English-speaking abilities, immigration status, and physical abilities in a multicultural environment.
