Job Description - LVN II - Prenatal Diagnostic Center - 8-Hour - Full Time - Days
Description Under the general direction of a licensed caregiver, the Licensed Vocational Nurse II is responsible for identifying the needs of each patient and assisting in the planning and delivery of patient care. After appropriate educational preparation, it is capable of an expanded scope of care in the areas of nursing process and intravenous therapy. Maintains competency skills including the ability to provide care to special patient populations and patients with diverse cultural backgrounds. It can identify the ability of the patient to follow directions/instructions. It can identify safety concerns and institutes additional measures to provide a safe environment and identify developmental milestones.
Responsibilities
As coordinated by the RN, it delivers patient care to meet patient needs based on an age-related plan of care and outcome-oriented standards of care. Performs IV therapy/phlebotomy/skin tests. Accepts and transcribes MD orders within the scope of care.
Identifies the physical, emotional, social, and cultural needs of assigned patients and reports findings to the RN
Observes, reports, and documents the effect of preventive, diagnostic, supportive, and therapeutic interventions. Monitors diagnostic results and reports to the RN
Provides patient instructions regarding patient safety, personal hygiene and procedures/treatments performed, as well as post-discharge preparation in collaboration with the RN
Integrates appropriate data sources to assess the signs, symptoms, reactions, behaviors, or general appearance to determine normal versus abnormal characteristics.
Communicates comprehensive patient care information to members of the clinical team as well as patients. Actively participates in unit rounds.
Provides and documents patient, family, and significant other education, including smoking cessation, pneumococcal vaccination, and other patient-specific subjects.
Provides care to special patient populations and patients with diverse cultural backgrounds. Identifies the patient’s readiness for learning and the ability to follow directions/instructions and give consent.
Under the direction of the charge nurse or primary RN, will safely use restraint interventions
Performs therapeutic communication, re-direction, reality orientation, the effective motivation of patients to participate in activities, and adequate conduction of rounds and observations to assure safety. Takes responsibility for reporting problems and/or patient complaints.
Provide patient safety and comfort by utilizing resources and materials to safely transport patients, answer patients' call lights and requests, and report observations of the patient to the nursing supervisor.
Ensures environmental cleanliness in inpatient rooms and patient care areas.
Qualifications Education
High School Diploma/GED - minimum
Experience
1 year 1 year acute care experience - minimum
Licenses and Certifications
Licensed Vocational Nurse - Upon Hire minimum California State Vocational Nurse License
Basic Life Support (BLS) - Upon Hire minimum BCLS Certification
