LVN / LPN Outpatient

Overview
We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our healthcare team. The ideal candidate will possess a strong commitment to patient care and demonstrate proficiency in various nursing practices. As an LVN, you will play a crucial role in providing high-quality care to patients in diverse settings, including hospitals, clinics, and emergency rooms. Your expertise will contribute to the overall well-being of our patients and support the healthcare team in delivering exceptional service.

Responsibilities

• Provide direct patient care under the supervision of registered nurses and physicians.
• Administer medications and treatments as prescribed, ensuring compliance with safety protocols.
• Monitor patient vital signs and report any changes or concerns to the healthcare team.
• Assist with medical procedures, including tube feeding and wound care.
• Maintain accurate medical documentation and coding, adhering to ICD-9 standards.
• Collaborate with case management teams to develop and implement patient care plans.
• Educate patients and their families on health management and post-discharge instructions.
• Ensure a clean and safe environment for patients by following infection control guidelines.

Years of Experience: 2 year(s)

Certfications: BLS, IV Certified

Charting System: Required, Epic

Experience

• Previous experience in an emergency room, medical-surgical unit, operating room (OR), or clinic setting is preferred.
• Familiarity with medical documentation practices and coding is advantageous.
• Strong communication skills and the ability to work effectively within a multidisciplinary team.
• A valid LVN license in the state of practice is required.
• Experience in case management is a plus but not mandatory.

Pay: $42.25 per hour

Expected hours: 36 per week
