JOB SUMMARY: To provide nursing care to residents as prescribed by the physician and in accordance with standards of nursing practices, regulations and directed by the Director of Nursing. To lead or direct licensed and non-professional staff in the delivery of direct Resident care and support functions.
ESSENTIAL FUNCTIONS:
Utilize working knowledge of the basic principles of infection control and wound care
Demonstrate competence in providing wound care that is sterile and/or aseptic
Apply knowledge and understanding of the special needs of the geriatric client and the geriatric client’s skin
Remain free of any communicable disease and must maintain hands with clean, non-jagged nails which are of appropriate length to assure residents are cared for in a risk free manner
Provide wound care (bandaging, cleaning, applying medication) and assess for appropriateness of bandage integrity during time between dressing changes
Assure availability of treatment supplies and topical medications (as prescribed by the physician) and procure supplies to assure adequate inventory or Treatment Cart is readily available at all times
Provide gastrostomy tube feeding care, including necessary bandage changes
Provide colostomy and urostomy care and dressings as directed by the physician
Provide weekly documentation
Comply with facility guidelines for documentation and physician’s orders
Relate well to patients, families and other team members
Interact well with therapists and implements individual programs as developed
Maintain equipment and supplies for treatment
Maintain consistent patient care schedule and uses time efficiently
Coordinate restorative treatments with other facility departments
Perform duties with a minimum of supervision
Identify potential problems/issues and report to supervisor in a timely manner
Maintain a professional approach with confidentiality
Supportive facility goals and objectives
Use system to convey/receive resident information
Cooperate and maintain good rapport with nursing staff, medical staff, other departments, and visitors
Obtain and document nursing history from the patient and/or significant other that is comprehensive of present condition and prior problems
Perform basic assessment of the patient, indicating adequate knowledge of anatomy, physiology, and pathology
Record vital signs, note changes, and pursue more-specific investigation as needed
Assess for and notify physician and other appropriate parties of changes in condition
Use assessment information to develop a care plan before the end of duty time that communicates enough information for incoming personnel to adequately care for the patient? Use care plans as a basis for providing safe and therapeutic care to patients
Consistently follow established standards, policies, and procedures in providing nursing care
Accept accountability for clinical care of assigned patients, including supervision of nonprofessional personnel
Communicate with the patient and his or her family and other members of the health care team in carrying out the goals of the care plan
Perform competently those technical skills considered basic to the type of care given on that unit
Respond appropriately in urgent and/or emergency situations
Support standards of nursing care through adherence to existing policies and procedures
Seek guidance when encountering nursing procedures and responsibilities that have not been mastered
Promote positive public relations with patients, residents, family members and guests
Use tactful, appropriate communications in sensitive and emotional situations
Use appropriate workplace behavior and adhere to dress code at all times
Understand and adhere to the guidelines of “Residents Rights” and assure resident safety
Requirements
QUALIFICATIONS:
Required
Current LVN license
CPR certified required; 30 days will be given to complete a course and get certified
Ability to understand, remember and carry out verbal or written instructions in English
Ability to complete assignments in a timely manner
Ability to work flexibly and positively with intermittent interruptions Preferred
2 years nursing experience in long term care/skilled nursing
PHYSICAL REQUIREMENTS: The ability to safely perform movements such as pushing, pulling, lifting, bending, kneeling, and reaching up to 50 pounds with or without reasonable accommodations
WORK SCHEDULE: As assigned, including some weekends, evenings and holidays
WORK ENVIRONMENT: Clinical
