LVN/LPN Pediatric Home Health

Open ShiftsJob Summary:

Provides general care and education to patients under the direction of a RN or BSN. Performs tasks including scheduling, record-keeping, and maintaining supply inventory

Essential Job Functions:

• Takes patient vital signs
• Performs common tasks, including but not limited to:
• Treating bedsores
• Dressing wounds
• Bathing
• Dressing
• Coordinates visits and paperwork for patient/doctor visits.
• Record the client’s progress and conditions
• Records and reports symptoms.
• Reviews patient charts and records essential information, including medical past and history, reactions, treatments, etc.
• Assists patients with daily hygiene needs.
• Maintains inventory and stock of clinical and medicinal supplies.
• Educates patients on current health issues.
• Practices safety and health code standards.
• Performs jobs assigned by head nurses and doctors as directed.

Job Qualifications:

Educations: Degree as a Licensed Vocational/ Practical Nurse

Licensure: Current Texas State License as an LVN/LPN, current Texas Driver’s License.

Experience: 6 months experience in acute care or rehabilitation setting, home health preferred.

Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.

Transportation: Reliable transportation and valid and current auto liability insurance.

Environmental and Working Conditions

Works in a patient’s home in various conditions; possible exposure to blood and bodily fluids and infectious diseases; ability to work flexible schedule; ability to travel locally; some exposure to unpleasant weather.

Physical and Mental Efforts:

Requires working under some stressful conditions to meet deadlines and patient needs and to meet patient/family individualized psycho social needs.
