Job Description

General Summary

 

In collaboration with Director of Integrated Care Management, manages the operations of the Care Management Department to include Care Management, Discharge and Transition Planning and Utilization Management in accordance with regulatory requirements and the needs of the hospital. Assumes shared responsibility for ensuring optimal clinical and financial outcomes across provider lines and systems in conjunction with all members of the healthcare team.

Qualifications

 

Education and Experience

    1. One of the following required: Bachelor of Science in Nursing or Master of Social Work.  
    2. One of the following required: Current RN license in State of Maryland or current Maryland Licensed Clinical Social Worker (LCSW).
    3. Minimum five (5) years of previous case management experience required which must include at least two (2 years of previous supervisory experience.

 

Knowledge, Skills and Abilities

  • Is responsible for contributing to the development and administration of departmental goals and objectives and their concurrence with the hospital mission, vision and goals.
    • Understands the bigger picture and his/her role in it.
  • Is responsible for ensuring the delivery of high quality service to all customers, as well as meeting regulatory requirements and continually improving performance.
  • Is responsible for appropriate management of resources.
    • Development and administration of annual fiscal goals, including operating and capital resources.
    • Effectively manages the human resources of the department. Includes managing staffing levels, developing staff, maintaining good communication, and promoting high employee morale.
    • Ensures adequate staffing levels for hospital operations through proactive recruitment and retention efforts.
  • Demonstrates leadership abilities consistent with the goals and values of the organization.
    • Communication skills:
      • Utilizes appropriate customer relations skills.
      • Discourages gossip and seeks clarification to effectively communicate accurate information.
      • Keeps senior management abreast of pertinent information.
  • Professionalism
    • Demonstrates responsibility for self-learning and professional growth..
    • Maintains confidentiality of patient, employee and hospital information.
    • Maintains professional attitude and appearance.
    • Assumes responsibility and is accountable for own actions. Supports hospital decisions.
  • Maintains effective relationships with the medical staff.
  • Functions as a collaborative team member, demonstrates commitment to group goals and teamwork.
  • Manages change effectively, including being flexible and adaptive.
  • Utilizes critical thinking to identify problems proactively, and independently generates solutions/resolutions as necessary.
  • Consistently demonstrates integrity, resilience and perseverance in this leadership capacity.
