Job Description
What You Will Be Doing

  • Oversee the daily operations of the Care Management Department, including care coordination, case management, discharge planning, utilization management, social work services, and pre-admission case management.

  • Lead and support care management staff to ensure high-quality patient care and effective discharge planning.

  • Establish and implement best practices, clinical standards, and departmental policies to improve care coordination and operational efficiency.

  • Serve as a liaison between patients and families, physicians, healthcare teams, community agencies, and managed care organizations to ensure coordinated care delivery.

  • Monitor department performance and ensure compliance with clinical, regulatory, and organizational standards.

  • Collaborate with leadership to maintain financial integrity and effective resource utilization.

  • Build and maintain relationships with skilled nursing facilities, community partners, and external healthcare providers to support smooth patient transitions.

Experience You Will Need

  • Master’s Degree in Social Work (MSW) from an accredited program.

  • Washington Clinical Independent Social Worker (LICSW) License required.

  • 6+ years of experience in Case Management in an acute care setting.

  • 2+ years of leadership experience at the Manager or Director level, preferably in case management within a large hospital.

  • Strong leadership, communication, and team development skills.

  • Experience collaborating with multidisciplinary healthcare teams and external care providers.

  • Knowledge of care coordination, discharge planning, and utilization management practices.
