Manager Case Management (RN or MSW)

1. Oversee the daily operations of the case management department and ensure efficient and effective delivery of services.


2. Develop and implement policies and procedures for case management services in accordance with organizational standards and regulations.


3. Hire, train, and supervise a team of case managers, providing guidance and support to ensure high-quality performance.


4. Collaborate with other departments to develop and implement strategies for improving patient outcomes and achieving organizational goals.


5. Monitor and analyze data related to case management services, identify areas for improvement, and implement solutions to enhance efficiency and effectiveness.


6. Ensure compliance with all applicable laws, regulations, and standards related to case management services.


7. Serve as a liaison between the case management department and other healthcare providers, community organizations, and insurance companies.


8. Develop and maintain relationships with external stakeholders to enhance the quality of care and resources available to patients.


9. Lead and participate in interdisciplinary meetings to discuss patient cases and develop care plans.

Education Qualifications

• Bachelor’s Degree in nursing Required or


• Master’s degree in social work Required

Experience Qualifications

• Minimum of five years' relevant experience of hospital experience recommended. Required


• Minimum of three years' relevant experience of case management supervisory experience Preferred

Licenses and Certifications

• Current RN Required or


• Social work licensure in applicable state. Required


• Certification in Case Management Required or


• plan in place to obtain within 2 years of hire date

Hours: 40per weeks

Schedule: Monday-Friday
