Summary:
The Manager, Data Insights & Population Health is responsible for implementing and overseeing strategic programs that support payment and care delivery transformation. The Manager will support the Director on formulating of data insights strategy to support population health initiatives. The Manager is responsible for ensuring the achievement of all program performance goals and targets across the Contexture enterprise by providing program coordination and accountability. The Manager works collaboratively with Contexture leadership, and cross functional units to support effective population health data strategies with all Contexture HIE services and other Contexture service lines. The Manager is responsible for the day-to-day management of the Quality Improvement (QI) Advisors
This position is based in Phoenix Arizona; Denver Colorado; or Grand Junction, Colorado and requires local residency in one of these base locations. Our strategic flexibility allows for local work from home opportunities.
Essential Duties and Responsibilities include the following:
- Assists the Director in the implementation of operational plans, strategies, tactics, performance goals and deliverables for value-based programs, such as but not limited to the AHCCCS Differential Adjusted Payment Program (DAP) and HCPF electronic clinical quality (eCQM) program and population health insights.
- Work with Contexture QI team to support and direct the team to monitor data and validate data reports, understand quality improvement, to develop practice facilitation and technical assistance roadmaps and provide direction on performance improvement techniques, including:
- Analyzing data quality report results.
- Validating clinical quality data.
- Setting measurable goals.
- Identifying trends and opportunities for intervention from data.
- Communicating with EHR vendors.
- Mapping clinical quality data within the EHR.
- Measuring and auditing large volumes of data for quality issues and improvements.
- Escalating data quality issues, working with teams within Contexture and with stakeholders.
- Evaluating and quantifying business impact of data quality issues and making recommendations for data improvements including required process and system changes.
- Oversees the analysis of data to extract valued insights and support decision-making process, turn data into actionable information.
- Implement and maintain operational processes for data solution product management in alignment with data solution strategy.
- Design, document, and maintain SOPs for product management.
- Oversees the annual programs’ submission process at Contexture, including outbound communications, logging, and tracking and reporting functions. Trains and coordinates with internal teams to ensure accurate and consistent responses to stakeholder questions regarding programs and processes.
- Coordinates with Marketing to develop effective outbound communication campaigns and materials, including but not limited to Notices, Alerts, webinars, FAQs, and other community presentations.
- Work collaboratively across the data management team and other technical teams to identify and support a strategy of progressive targets and milestones, within all managed programs, improving the overall quality of data submitted to the HIE.
- Implement the defined strategy for obtaining performance feedback and scorecards to improve Contexture responsiveness and timeliness and support overall improvement of the designated programs at Contexture.
- Deliver technical assistance, training, and coaching to healthcare providers and staff on quality improvement methodologies, tools, and best practices.
- Participate in knowledge exchange, practice facilitation and technical assistance initiatives to improve the quality and performance of participating agencies.
- Collaborate with all Contexture service lines leadership and teams to support program goals with related milestones.
- Coordinate with business development team to create advanced education and training programs and curriculum to support provider/practice adoption of HIE services, tools, and advanced applications in healthcare integration. Provide internal staff training and education to support knowledge exchange and employee growth.
- In-depth understanding of Health Care markets, including local, state, and federal programs and grant programs, and their use of health information technology and data elements needs.
- Provide daily supervision and management of assigned staff, maintain allocated staffing levels including leading recruitment efforts, training, and creating on-boarding plans for new or transferred employees.
- Ensure adherence to Scrum framework and support Agile transformation initiatives.
- Perform other related duties as assigned.

Skills
- Strong understanding of HL7, C-CDA and XDS.b standards and protocols, and HIE standards and protocols.
- Strong knowledge of Electronic Health Records.
- Robust knowledge of healthcare clinical codes sets such as SNOMED, LOINC, CPT, ICD, RxNorm, etc.
- Experience with quality assurance and quality improvement methodologies, techniques, tools and in implementing data quality principles and processes.
- Understanding of provider/practice operational and business needs in the day-to-day environment including integrated care and advance coordination models.
- Understanding of hospital operational and business needs in the day-to-day environment.
- Experience and/or understanding of complex care programs and measurement-based care, including use of data to support business and clinical objectives.
- Experience in Value Based and Alternative Payment Models.
- Experience with training and education programs and development of user tools in a variety of applications.
- Experience in developing and presenting informational presentations to small and large constituent audiences.
- Strong knowledge of practice facilitation and quality improvement techniques such as process mapping, fishbone diagrams, and PDSAs.
- Experience providing practice facilitation, technical assistance, or consulting services to healthcare providers.
- Proficiency with the Microsoft Office Suite including Excel, Word, PowerPoint, preferred.
- Strong understanding of Agile and Scrum methodologies.
- Proven ability to lead cross-functional teams and manage stakeholder expectations.

Education/Experience
Minimum of 5+ years of direct healthcare delivery or oversight of healthcare delivery with a Bachelor’s Degree in a behavioral health, applied health or healthcare-related field or commensurate directly related work experience. 3+ years’ experience with healthcare technology including EHR and healthcare data elements (such as HL7 or CCD standards). Broad experience in multiple sectors and levels of healthcare. Experience preferred in AZ and CO State Medicaid programs and/or CMS Programs. Prior management experience preferred. Experience in healthcare administration and program evaluation preferred. Advanced certification in practice management or other national standards (CPHQ, NCQA etc.) preferred.
