InterCare is searching for a Full-Time Manager of Care Management Services to join our team! Work Hours

This is an on-site (remote flexible), Monday - Friday from 8:00 a.m. - 5:00 p.m. position.

Minimum Qualifications

  • Current RN license in the State of Michigan.  
  • Possesses a thorough understanding of theory and practices of Care Coordination and Population Management typically acquired through completion of a bachelor’s degree program in nursing and extensive practical experience in a professional environment.

Primary Accountability

Responsible for the oversight and support of the Care management service line to include: care management, care coordination  and population health activities In addition, is  responsible for the care management service line in relation to expectations from Value Based Contracts(VBC), and organizational productivity.

Description of Primary Duties & Responsibilities

1. Maintains high performing teams and effectively supervises assigned employees.

  • Hires, directs, coordinates, and coaches assigned employees.
  • Clearly sets,communicates, and monitors performance and productivity  expectations.
  • Regularly provides constructive feedback to assigned employees and conducts timely and accurate performance evaluations.
  • Identifies and addresses service lines staff’s training and development needs.
  • Aligns direct reports with organizational priorities and maintains a strong focus on their engagement and accountability.
  • Facilitates excellent employee communication pathways.
  • Oversees uniform implementation of HR policy and procedure.

2. Serves as internal consultant for care management, care coordination/population health management processes including organizational support to the Care Managers.

  • In coordination with MIS and EHR clinical develops documentation processes for assigned staff  including work flow and templates.
  • Serves as internal advocate and expert for care management, care coordination,  population health  and VBC.
  • Provides oversight of the Care Management process, including billing process and review of records for accurate documentation
  • Exhibits awareness of financial indicators related to Care Management and Population managements.
  • Leads monthly Care Manger/care coordinator meeting. Provides ongoing education in the area of best practice care management.
  • Promotes care management, care coordination, population management and integration. 
  • Writes policies and procedures as necessary for care management and population management 
  • Works collaboratively with site managers to assist staff in creating a leadership team and care model that will implement measures required to improve chronic care.

3. Provides ongoing education in the areas of responsibility.

  • Provides ongoing education in the areas of Care Management and Population management to staff.
  • Responsible for the orientation of Care Management/ Care Coordination staff.
  • Updates providers as needed regarding areas of responsibility.
  • Demonstrates knowledge of the financial incentives and timelines 
  • Expresses awareness of HEDIS measures and status of current financial environment of managed care programs. 

4. Provides Care Management to identified patient population to include:

  • Population Health
  • CCM( chronic care management) and PCM( principle care management): See CM job description
  • Provides patient education

5. Supports ongoing work in Quality Improvement.

  • Researches, gathers and reports health related data to meet requirements of NCQA model
  • As a member of CSD participates in, UDS audits and other duties as assigned.
  • Participates in program evaluation and quality improvement activities. 
  • Participates, facilitates and leads other initiatives that align with the PCMH model.

  • Maintains a working knowledge of EHR templates and workflow.

Technical Skills

  • Proficient in Microsoft Office Suite
  • Ability to use clerical and numerical skills in preparing final drafts and documents.
  • Ability to create letters, forms, documents, presentations.
  • Ability to run reports, analyze and interpret data, and assist in implementation.

Communication Skills

  • Possesses a professional level of written and verbal communication skills
  • Ability to communicate complex concept in a clear effective manner
  • Possesses excellent cross cultural communication skills and the ability to communicate to staff members at all levels
