Job Description - Manager of Case Management

Description:

Summary

Oversee the de l ivery of consistent, high qua l ity and patient centered care, promote the professiona l

growth and deve l opment of the Case Managers and C l inica l

Socia l

Workers, and deve l op operationa l

efficiencies.

Ensure case management practices comp l y with department, hospita l , and regu l atory po l icies, procedures and professiona l

standards of practice.

Deve l op, imp l ement, and maintain Case Management Department Standards of Practice and Po l icies and Procedures.

Serve as a

l iaison and resource for the Program Directors, Nurse Managers, Therapy Directors, Physicians and other c l inica l

and non-c l inica l

services and departments within the organization in the areas of case management and socia l

services.

Participate in qua l ity improvement and program deve l opment activities. Participate in deve l opment and assurance of genera l

and program specific competencies as it re l ates to the patient popu l ation served.

outcomes.

Comp l iance audits are conducted to ensure adherence to department standards of practice, po l icies and procedures.

Department qua l ity improvement projects are active l y engaged in to achieve estab l ished objectives.

·

Work assignments are distributed equitab l y among team members and meet the needs of the department.

·

Staffing schedu l e is deve l oped and maintained to ensure coverage meets anticipated needs.

·

l ength of stay patients are monitored to reso l ve barriers to discharge and support is provided to team members to achieve

l ength of stay targets.

·

Denia l s and appea l s are reviewed within estab l ished timeframes.

·

Estab l ished department goa l s are achieved through active participation in Uti l ization Review Committee.

·

Case Management Department Standards of Practice, Po l icies and Procedures are deve l oped and imp l emented in co l

l aboration with Case Management Director.

Additional department, organization, or network activities are completed per established objectives.

Qualifications:

Qualifications and Experience

Experience

Required

·

Minimum of five years supervisory or management experience with professional staff in a healthcare setting.

Preferred

·

Five years of related rehabilitation experience and certification in case management preferred.

Case management, uti l ization review, or discharge p l anning experience in an acute, rehabi l itation, behaviora l

hea l th, ski l

l ed nursing, or home care is necessary required. Health plan experience in utilization review and case management will also be considered.

Licensure

Required

·

Current license to practice in Massachusetts in Nursing, Physical Therapy, Occupational Therapy, Speech/Language Pathology, Social Work, or Masters in Rehabilitation Counseling.

Skills

(Specific learned activity gained through training (e.g. computer skills, keyboarding, presentation, CPR, ACLS,

etc.)

Required

·

Knowledge of community services and the health care system is required. Knowledge of medical review criteria, and levels of care is required.

·

Computer proficiency required.

Microsoft office applications preferred with ability to learn new software.

Preferred

·

Highly developed communications and interpersonal skills, working with diverse population.

·

Work independently, be self-directed and contribute as a member of a team.

·

Anticipates challenges and develops and implements strategies for addressing them.

·

High level of service delivery. Demonstrate initiative with ability to prioritize work, meet deadlines and adapt to changing situations.

·

Attention to detail.
