Job Description:

Job Summary


Responsible for demonstrating expertise in skilled clinical nursing to coordinate comprehensive care for homebound patients. Oversees the delivery of patient-centered care to ensure quality and continuity. Collaborates with interdisciplinary teams to develop and implement individualized care plans. Monitors patient progress and adjusts care strategies as needed. Ensures compliance with clinical standards and regulatory requirements.




Essential Functions


• Manages and coordinates the referral and intake process for patient care assignments.


• Collaborates with Intake LPN to assign case managers and requested disciplines.


• Communicates referral and intake activity to relevant staff and maintains intake log in the EMR.


• Coordinates patient care and makes staff assignments, including float and PRN nursing staff.


• Takes administrative calls and coordinates therapy assignments for new referrals.


• Performs office nursing duties, including taking MD orders and tracking SOC and F2F returns.


• Assists with orientation, training, and competency evaluation of nursing staff.


• Provides day-to-day nursing supervision, serves as an expert resource, and supports staff development.




• Performs other duties as assigned.




Education Requirement


Bachelor’s degree in nursing, healthcare administration, public health, social work, or a related field






Experience Requirement


4-5 years of relevant/progressive experience in clinical experience, or a relevant area




An equivalent combination of education and experience may be considered.


All experience must be paid and in the same related field.


Part-time and PRN experience will be prorated based on hours worked per week.


Volunteer work and internships for academic credit are not counted.




Certifications & Licensures


Current and active Registered Nurse (RN) license
