Master of Social Worker

AdvisaCare Home Health & Hospice is seeking a compassionate and dedicated Social Worker Per diem to join our team. In this role, you will provide critical support to patients and their families by addressing psychosocial needs, offering counseling, and connecting clients with appropriate community resources. You will be part of a multidisciplinary team aimed at enhancing the quality of life for individuals receiving home health and hospice care.

• Master's degree in Social Work (MSW) from an accredited program
• Current licensure as a Social Worker (LMSW or LCSW) in the state of practice
• Experience in home health, hospice, or related healthcare settings is preferred
• Strong assessment, counseling, and case management skills
• Excellent communication and interpersonal abilities
• Valid driver's license, reliable transportation, and car insurance

Additional requirements may include successful completion of background checks and drug screenings as per company policy.
