Join the dedicated LIFE Senior Services PACE (Program of All-Inclusive Care for the Elderly) team, where we provide essential care to Tulsa's aging population. Our team offers participants access to comprehensive medical services and social support while they continue to live independently in their own homes and communities. Many of our participants face multiple chronic conditions and have had limited access to healthcare in the past.

We are seeking a Master’s Social Worker (MSW) to expand our staff and join a team of professionals who excel in providing specialized care for seniors. The MSW’s primary role is to connect PACE participants and their caregivers with community resources to meet their diverse needs. You will collaborate closely with an interdisciplinary team of physical therapists, healthcare providers, social workers, and nurses to ensure our participants receive the highest level of care.

Your responsibilities will include identifying and facilitating resources to address participant needs such as housing, mental health services, and caregiver support. This role is dynamic and evolving, as new needs frequently arise from participants and their families. You will also participate in regular assessments and contribute to interdisciplinary care planning.

Working exclusively with seniors, including individuals with dementia, requires patience, empathy, and compassion. This position provides the unique opportunity to build long-term relationships with participants and their families, enhancing their quality of life.

Requirements

Education and Experience:

· Master’s degree in social work is required.

· Experience in working with a diverse, elderly population, preferable in a community-based setting or geriatric program.

· Knowledge of financing mechanisms such as Medicare, Medicaid, and Prospective Payment Systems.

· Ability to work effectively and harmoniously with the staff, the elderly, providers of services, and public and private agencies.

· Valid driver’s license, proof of vehicle insurance, clean driving record and use of personal vehicle.
