MAT Medical Provider

SUMMARY:

Under the direction and supervision of the Chief Medical Officer, the Medical Provider conducts comprehensive medical assessments and provides individualized care, including prescribing and adjusting medications as needed. Serving as a key member of the MAT team, the provider delivers services to clients both in-office and via telehealth across various access points. Responsibilities include patient evaluation, treatment management, and medication prescribing at SFRC Gallup facilities. The Medical Provider collaborates closely with the MAT team—comprising Case Managers, Client Engagement Advocates, and the MAT Coordinator—to support patients with substance use disorders and co-occurring behavioral health illnesses. Additional duties involve coordinating care with referral sources, offering educational guidance, and participating in interdisciplinary consultations with patients, families, caregivers, and staff to ensure holistic treatment and support.

DUTIES AND RESPONSIBILITIES:

· Review patients’ medical histories to ensure comprehensive understanding of their health needs and perform physical examinations to evaluate patients’ overall health.

· Assess, diagnose, and develop treatment plans for patients with a focus on co-occurring substance use and medical conditions.

· Provide brief treatment interventions, education, and counseling to patients and their families regarding diagnoses, treatment options, and health concerns.

· Prescribe and manage medications, including those for substance use disorders, adhering to program protocols.

· Document patient progress, treatment plans, and care coordination using the agency's EMR system.

· Guide and support MAT team in diagnostic and medication management for MAT clients.

· Participate in quality assurance initiatives and educational programs, including training on opioid addiction treatment, buprenorphine use, diversion control, overdose prevention, and comprehensive health management for addiction recovery.

· Performs buprenorphine induction, evaluation, management, and follow-up appointments; coordinating with SFRC Gallup staff to ensure efficiency and quality recovery services are provided for ongoing treatment and after discharge.

• Follows      medical protocols established by the Chief Medical Officer including      admission, treatment, and discharge standards in accordance with agency      policy and procedure.
• Participate      in required staff supervision, continuing education, and training.
• Assist with      Mobile Unit and Access points. 
• Performs any      related duties as required and assigned by the Chief Medical Officer.

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:

• Provide      compassionate patient care to individuals struggling with addiction and demonstrate      commitment to their chosen path to recovery.
• Abides by      the professional standards, rules, and regulations as promulgated by      relevant state and federal licensing boards.
• Abides by      agency policy and procedure established by the Board of Directors and      Executive Management.
• Proficiency in      providing medical and treatment information to program staff in person, by      telephone, in written form, or by e-mail.
• Ability to      make good decisions quickly and multitask well.
• Ability to      consult a physician when there are limitations in knowledge or      understanding of the concern/issue at hand.
• Ability to      be detail-oriented in order to relay information accurately to internal      and external treatment providers.
• Provide      education to clients with limited health literacy and to those with      English as their second language.

MINIMUM QUALIFICATIONS:

1. State of New Mexico Medical License (MD, DO, PA, or NP)
2. Possesses current DEA and NM-CSR licenses; 
3. Proficiency with office technology and equipment, such as computers, copiers, scanners, fax machines, and phone systems, required
4. Proficiency with MS Office applications, required
5. Proficiency with electronic health/medical records (EHR/EMR) documentation, required
6. Excellent interpersonal, verbal, and written communication skills
7. Possesses a values base consistent with the mission and philosophy of the Agency
8. Must have and maintain a current, valid NM driver’s license. Must also have and maintain a spotless driving record, as well as auto insurance
9. Minimum 3 years of experience working with co-occurring disorders, preferred

Bi-lingual/Bi-cultural preferred (English/Spanish) (English/Navajo), preferred
