SUMMARY:
Under the direction and supervision of the Chief Medical Officer, the Medical Provider conducts comprehensive medical assessments and provides individualized care, including prescribing and adjusting medications as needed. Serving as a key member of the MAT team, the provider delivers services to clients both in-office and via telehealth across various access points. Responsibilities include patient evaluation, treatment management, and medication prescribing at SFRC Gallup facilities. The Medical Provider collaborates closely with the MAT team—comprising Case Managers, Client Engagement Advocates, and the MAT Coordinator—to support patients with substance use disorders and co-occurring behavioral health illnesses. Additional duties involve coordinating care with referral sources, offering educational guidance, and participating in interdisciplinary consultations with patients, families, caregivers, and staff to ensure holistic treatment and support.

DUTIES AND RESPONSIBILITIES:
· Review patients’ medical histories to ensure comprehensive understanding of their health needs and perform physical examinations to evaluate patients’ overall health.
· Assess, diagnose, and develop treatment plans for patients with a focus on co-occurring substance use and medical conditions.
· Provide brief treatment interventions, education, and counseling to patients and their families regarding diagnoses, treatment options, and health concerns.
· Prescribe and manage medications, including those for substance use disorders, adhering to program protocols.
· Document patient progress, treatment plans, and care coordination using the agency's EMR system.
· Guide and support MAT team in diagnostic and medication management for MAT clients.
· Participate in quality assurance initiatives and educational programs, including training on opioid addiction treatment, buprenorphine use, diversion control, overdose prevention, and comprehensive health management for addiction recovery.
· Performs buprenorphine induction, evaluation, management, and follow-up appointments; coordinating with SFRC Gallup staff to ensure efficiency and quality recovery services are provided for ongoing treatment and after discharge.
