Job Summary:

Position Responsibilities & Essential Duties:

Provider Support

Prescription refill refills/requests

Maintains informative brochures and educational materials.

Contacting referring providers to clarify referral or request additional records prior to appointment

Help with coordination of reduction/termination procedures

Networks with referring providers to establish trusting relationships

New OB intake

Postpartum wound checks

Medication administration

Completion of FMLA/Short Term Disability paperwork for patients

Addressing patient questions via EPIC

Phone Triage

Speaks with patients that call the clinic with health questions and provides guidance.

Works with providers to address patient care needs over the phone.

Calls patients with testing results and health education.

Health Coaching

New OB patient teaching

Third trimester/Breastfeeding teaching

Injection teaching

Assists with patient care

Assists with quality improvement projects.

Assists with fellow orientation and learning

Documents patient care using the electronic medical record system.

Takes an active role in promoting and building the integrated MFM health team.

Takes on assigned projects as needed.

Qualifications:

Minimum Qualifications:

Graduate with bachelor's degree in nursing from an accredited school of nursing

Current RN licensure in good standing (any state) (Must have CO RN license by date of hire. Could be working toward CO clinical licenses during application process and registered with DORA-Department of Regulatory Agencies.)

3 years of experience with Maternal Fetal Medicine in an outpatient clinical setting

Preferred Qualifications:

Current BLS/CPR certification

Electronic Health Record (EHR) experience

Experience working in a Federally Qualified Health Center or public health setting

Experience providing case management to low-income families

Bilingual (conversational level in Spanish and English)

Skill and Ability Requirements:

Basic knowledge of Colorado state immunization requirements

HIPAA and OSHA compliance

Intermediate level computer competency

Exemplary organizational skills and ability to prioritize organizational needs

Ability to work independently and function collaboratively as a team member

Highly responsible, reliable, and courteous

This RN Care Coordinator should be team oriented and willing to uphold the Maternal Fetal Medicine mission to provide women with a full range of obstetric and maternal medicine expertise necessary to treat any complication a high-risk mother might face. The RN Coordinator provides high quality support to physicians and advanced practice providers (APPs) in meeting operational needs of the practice and the medical needs of the patient. The division of MFM strives to ensure that each patient's experience is met with a high degree of satisfaction. This position is responsible for providing patient care coordination, helping patients navigate the various services provided across the CU MFM care system, help families access available community services, and triage of patient phone calls throughout the day. The RN Care Coordinator will also fill the role of health coach, providing health education and critical support to high-risk mothers before, during, and after their pregnancy.

– this role is eligible for a hybrid schedule of 3 days per week on campus and as needed for in-person meetings.
