Under general supervision of the Director of Nursing provides care coordination and discharge planning for an assigned patient case load.  This role is patient focused, outcome-oriented, and based on general and specialty professional standards, and functions within a multi-disciplinary practice model.  This role is aimed for the nurse who is a clinical and professional leader. He/she advances evidence based practices, participates in unit governance initiatives, and mentors others to achieve and grow in the role of the professional nurse.  As a senior member of the practice partnership at the unit level, serves as a leader, change agent, and coordinator of patient care services within a specific service line.  Takes a leadership role in the patient centered care delivery system that facilitates achievement of patient outcomes, supports continuity of care and patient flow between care delivery sites, and enhances patient safety and care giver satisfaction.  Monitors the quality and cost effectiveness of nursing practice and governance systems at the unit level.  Is accountable patients and families for the processes and outcomes of care during an episode of care.  Is accountable to staff for the advancement of group practice on the unit, and to the Director of Nursing and the Nurse Manager for the totality of work performance. 

• Licensure as a Registered Nurse in the state of Maryland, or eligible to practice due to Compact state agreements outlined through the MD Board of Nursing, is required.
• Bachelors degree preferred (BSN preferred).
• Relevant clinical work experience related to practice on the unit.
• Knowledge of discharge planning process is required.
• Maintenance of 20 contact hours of education/year.
• Member of a professional organization.
• Demonstrated evidence of professional growth and development through enrollment in a degree program, specialty certification, teaching in formal hospital programs or outside of hospital, board member of professional association, etc.
