Solaris Healthcare Apopka is now hiring a full-time MDS Assistant Coordinator Licensed Practical Nurse ( LPN ) or Registered Nurse ( RN ) as a MDS Assistant, who is committed to providing the best life and care for our residents. The MDS Assistant is responsible for implementing the federally mandated assessment tool to identify needs / concerns of the resident and to coordinate a plan of rare to assist the resident toward optimal wellness based on the highest practicable level of functioning.

MDS ASSISTANT QUALIFICATIONS:

Active Licensed Practical Nurse ( LPN )/ Registered Nurse ( RN ) in Florida
Organizational Skills/Ability to Multitask
Knowledge of PDPM/Clinical Concepts
Ability to work with facility nursing management to identify and fix care documentation notes
