Job Description - MDS Coordinator



An MDS Coordinator (Minimum Data Set Coordinator) is a highly skilled nursing professional responsible for overseeing the comprehensive assessment process for residents in long-term care facilities. Utilizing the federally mandated MDS system, the Coordinator ensures precise documentation, facilitates effective care planning, and guarantees appropriate Medicare and Medicaid reimbursement. Collaborating closely with interdisciplinary teams, the Coordinator meticulously captures and evaluates each resident’s physical, mental, and psychosocial needs. This role is essential to maintaining regulatory compliance, enhancing the quality of care, and supporting the financial well-being of the facility.

Key Responsibilities




  • Oversee MDS Assessments: Coordinate and ensure the timely and accurate completion of the Minimum Data Set (MDS) for all residents, maintaining compliance with applicable standards.




  • Interdisciplinary Collaboration: Partner effectively with nursing staff, physical and occupational therapists, dietitians, and attending physicians to collect comprehensive data and develop holistic, person-centered care plans.




  • Care Planning: Utilize MDS data to formulate individualized care plans that respectfully address each resident's unique strengths, needs, and preferences.




  • Reimbursement & Compliance: Ensure all documentation complies with federal regulations, including OBRA guidelines, to facilitate accurate Medicare and Medicaid reimbursements, incorporating RUGs classifications and facility quality measures.




Qualifications



  • Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing, or vocational nursing

  • Current state license as an RN

  • RAC-CT(REsidnet Assessment Coordinator Certified) preferred but willing to train the right candidate. 

  • Proof of current CPR certification

  • Compassionate, friendly demeanor

  • Excellent communication skills

  • Must be able to pass a background check and fingerprint process
