Citadel Healthcare is seeking a MDS Coordinator for (facility). The MDS Coordinator conducts and coordinates the development and completion of the resident assessment in accordance with the requirements of the state and thae policies and goals of this facility.
MDS Coordinator Essential Job Functions:
Conducts and coordinates the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment, including the implementation of CAAs and Triggers.
Acts as back-up to send denial letters when residents no longer are being skilled for Medicare and falls below Skilled level of Care.
Review all redeterminations requested, prepare s for cases to be presented to ALJ from CMA.
Acts as back-up to nursing staff in providing clinical information for BOM to fax updates as needed to insurance companies.
Develop s , through independent skills and judgment, implement s , and maintain s an ongoing quality assurance program for the resident assessment/care plans. Develop s action plans to ensure same.
Participate s in facility surveys (inspections) made by authorized government agencies, make s effective recommendations on improvements or action s needed.
Obtain s the facility and resident QM reports monthly from the state website.
Ensure s that all assessments are completed and transmitted in a timely manner. Report s problem areas to the Administrator.
Ensures a current copy of the MDS instructions Manual is available to per sonnel completing portions of the MDS.
Work s with the interdisciplinary Care Plan Team in developing a comprehensive resident assessment and care plan for each resident.
Serve s on, participate s in, and attend s various other committees of the facility (e.g., Policy Advisory, Quality Assessment and Assurance) as required, and as appointed by the Administrator.
Provide s written and/or oral reports of the resident assessment/care plan functions as requested or directed by committees.
Participate s in functions involving discharge plans as may be necessary.
Evaluate s and implement s recommendations from established committees as they may pertain to the assessment and/or care plan functions of the facility.
Education and Experience Requirements:
Must possess, as a minimum, an Illinois Nursing Degree from an accredited college or university
Must have, as a minimum, two (2) years of experience as a supervisor in a hospital, nursing care facility, or other related health care facility
Must have, as a minimum, six (6) months training experience in rehabilitative and restorative nursing practices.
