Your Responsibilities:

  • Document care performed and observations of resident status in clinical records promptly.
  • Communicate with residents' physicians and complete all nursing documentation.
  • Participate in medication management to promote optimal safety and effectiveness for residents.
  • Administer medications, observe and report resident responses, and manage narcotic records.
  • Assist with or institute emergency measures for sudden adverse developments with residents, including cardiopulmonary resuscitation.
  • Manage MDS Assessments based on CMS guidelines.
  • Coordinate, record, update, and monitor all minimum data sets.
  • Collect and report data on resident encounters and inter-institutional problems, making recommendations for change when appropriate.
  • Facilitate communication between residents, family members, medical staff, administrative staff, and payor sources.
  • Ensure the timely development and completion of resident assessments (MDS).
  • Apply MDS/PPS rules using the Utilization Guidelines for every assessment.
  • Develop written care plans for each resident, including measurable objectives.
  • Understand the financial concepts and implications of RUG IV, PPS, and state case mix.
  • Ensure all MDS assessments and RUGs are accurate, completed, encoded, edited, printed, and exported per state and federal requirements.
  • Assist with staff education for supportive documentation, including late loss ADLs.
  • Review quality metrics with clinical leadership to monitor trends for quality assurance process improvement action planning.
  • Monitor quality reporting program data to validate accuracy and avoid payment rate penalties.

Requirements:

  • All employees must pass state-specific background checks before contact with patients/residents.
  • Must have CPR certification upon hire or obtained quickly upon hire. CPR certification must remain current during employment.

Requirements

Education

Graduate of LPN program.

Licenses/Certification

Must have an active State issued LPN license.

NOTE: If the MDS Coordinator is an LPN, the MDS assessments must be reviewed by and signed off by the facility’s Chief Nursing Officer (RN).

Experience

Six months experience in a long-term care environment preferred.
