What You'll Do:

• Oversee the Resident Assessment Instrument (RAI), care planning, and PDPM processes to ensure regulatory compliance and optimal resident care
• Conduct and review MDS assessments
• Manage OBRA and Medicare schedules
• Support interdisciplinary care planning
• Participate in utilization review and quality assurance
• Collaborate within a highly successful and team-oriented environment

What You'll Need:

• Licensed Practical Nurse (LPN) in good standing in the State of Florida (required)
• Minimum three to five years related experience and/or training; or equivalent combination of education and experience
• At least 3 years of experience in SNF MDS management (required)
• Proficient in Microsoft Office including Word, Excel, and Outlook (required)
• Previous experience with RAI/CP/PDPM process (required)
• Excellent verbal and electronic communication skills, strong organizational skills, and ability to work well on a team
