We are seeking a highly motivated and experienced MDS Coordinator to join our team. The MDS Coordinator will be responsible for coordinating and completing the Minimum Data Set (MDS) assessments for our patients. The ideal candidate will have a strong attention to detail, excellent communication skills, and the ability to work well in a team environment.

Responsibilities:

- Coordinate and complete MDS assessments for patients

- Ensure accuracy and completeness of MDS assessments

- Collaborate with interdisciplinary team to develop care plans for patients

- Monitor and track patient progress and update care plans as needed

- Ensure compliance with all state and federal regulations related to MDS assessments

- Works closely with the nursing team, included in on call rotation schedule.

Requirements:

- Current Illinois LPN license

- Minimum of 2 years of experience in MDS coordination

- Knowledge of MDS assessment process and regulations

- Strong attention to detail and organizational skills

- Excellent communication and interpersonal skills

- Ability to work well in a team environment
