MED SOCIAL WORKER-MSW-SUPP

SOCIAL SERVICES - MSW\n\nPRN/VARIED HOURS/8 HOURS PER PAY\n\nFacility: Upper Valley Medical Center\n\nProvides direct and indirect clinical social work and discharge planning services for patients and their families in various inpatient and outpatient settings. Participates as a professional team member in patient care planning along with other hospital and community agency personnel. \n\n \n\nFrequent contact with patient care services staff, physicians, and ancillaries to discuss patient care and diagnostic needs. Participate in care rounds and other interdisciplinary collaboration within the in-patient and outpatient programs. Frequent contact with community agencies, third party payers, outside review organizations and case managers. \n\n* Master\u2019s in Social Work \n\nLicensure/Certification/Registration \n\n* LSW required; LISW preferred \n\nExperience \n\nMinimum Level of Experience Required: 1 - 3 years of job-related experience of job-related experience \n\nPreferred experience: Previous hospital or community social work experience required. Knowledge of community agencies preferred \n\nOther experience requirements: Excellent interpersonal skills, critical thinking and ability to communicate in writing. Excellent organizational skills. Ability to work in a busy, stressful environment. \n
