GENERAL DESCRIPTION

This position is part of the Diabetic team. This position is responsible for the initial interactions with patients, such as escorting patients, taking vital signs, setting up and breaking down exam rooms, injections, labs and other related duties. Responsible for moving patients through the clinic efficiently and effectively. Communicates with customers, employees, and other individuals to disseminate or explain information. – This position requires travel.

Requirements

ESSENTIAL DUTIES

  1. Rooms patients, collects and documents vitals, health history, social history, medication list, allergies, and any other documentation required for the medical visit in EMR.
  2. Performs any injections, education, or other tasks as requested by provider during patient visit.   
  3. Prepares treatment rooms for examination of patients; assures all rooms are clean and dispose of trash in rooms.
  4. Cleans and sterilizes instruments. Inventories medical supplies and materials.
  5. Maintains consistent patient flow by monitoring scheduling system.
  6. Transports specimens, equipment or supplies utilizing appropriate and safe techniques.
  7. Conducts follow up communication with no-show patients and/or patients receiving new diabetic management instructions and communicate with providers as appropriate. 
  8. Collects food/blood sugar logs from patients. 
  9. Inquires and informs diabetic patients of other healthcare services such as vision, dental and podiatry services. 
  10. Provides patient education on concerning glucose monitoring values and/or how to use a glucometer or CGM. 
  11. Provides instruction on insulin storage (vials, pens). 
  12. Provide information on use of Glucagon sources for emergency treatment. 
  13. Collect patient data as needed for reporting purposes. 
  14. Assist nursing staff in getting supplies/samples for diabetic management patients. 
  15. Keep diabetic management supplies on hand. 
  16. Complete trainings offered to further knowledge of diabetes. 
  17. Performs all duties, services and documentation in full compliance with CHC/SEK policies.
  18. Performs other related duties as may be assigned. 

QUALIFICATIONS

  1. High School diploma or equivalent.
  2. Graduation from a certified CMA/CNA program is preferred.
  3. Experience in clinic or healthcare setting.

KNOWLEDGE, SKILLS AND ABILITIES

  1. Social and cultural sensitivity appropriate to ethnically and economically diverse patient-and employee-base
  2. Demonstrate knowledge of the rationale of appropriate patient care.
  3. Communicates through appropriate channels. Use proper chain of command for patient complaints.
  4. Ability to handle emergency situations calmly and effectively.
  5. Must be computer literate, especially with Microsoft Office products and be able to use the Internet.
  6. Must be able to maintain good inter-personal relationship with co-workers and other members of the health care team and the organization.
  7. Provide customer service in accordance to the organization’s mission.
  8. Be courteous and respectful when interacting with patients and family members.
  9. Maintain patient confidentiality in accordance to organization’s policy and procedure and HIPAA requirements.

WORKING CONDITIONS

While performing the duties of this Job, the employee is regularly required to use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The employee is frequently required to stand; walk and sit. The employee must occasionally lift and/or move up to 10 pounds. Specific vision abilities required by this job include close vision, peripheral vision and ability to adjust focus. The noise level in the work environment is usually moderate. Hazardous material inventory and MSDS available. Wears personal protective equipment as needed. May be exposed to patients/public with communicable diseases.
