Make an impact. Join our growing outpatient clinics, focused on improving the wellbeing of families. We are seeking a mission driven and dedicated Assistant Case Manager for our health center in Anaheim.

• Bilingual (English/Spanish)

• Committed to providing high-quality services to underserved patients in Orange County.

• Willingness to work as part of a collaborative team that includes, social worker, physicians, nurse practitioners, and psychiatrist.

• Ambulatory (outpatient) care

• Daytime work schedule - Monday through Friday 8AM - 5PM.

Who We Are. We are a collaborative community practice, with locations in Fullerton, Anaheim, Orange, and the High Desert. We believe that everyone should have access to high quality comprehensive medical care, regardless of their ability to pay. Our health centers use a team-based model of care that includes family medicine, obstetrics, women’s health, oral and vision care, preventative and educational health, mental health, patient advocacy, case management, and community health. Most of our patients live below 200% of the Federal Poverty level, and are best served in Spanish. Through our origins with St. Joseph Health system, we have provided care to low-income families since the 1980s, maintaining a commitment to serving our most vulnerable neighbors with high-quality, nationally recognized care. Essential Functions (What You Do)

  • The Assistant Case Manager performs the functions of a Medical Assistant in addition to engaging in support of patients through undertaking core case management functions, including
  • Administering social drivers of health assessments
  • Aiding patients in the completion of paperwork and applications for assistance
  • Connecting patients to community and health related resources.  
  • The Assistant Case Manager helps patients set and work towards reasonable goals aimed at improving patients’ health and wellness.    

Pay Range: $23.00to $27.30 per hour

Requirements

Education:
Must have a certificate of completion from an accredited school of training for medical assistants. 

License/ Certifications:
Provider level basic life support (BLS) from AHA approved program. 

Valid Driver’s License. 

Experience: 

Minimum of 2 years of experience as a Medical Assistant or equivalent.  

Minimum of 1 year of care coordination, navigation or coaching experience. 

Community Clinic experience (Preferred) 

Experience with EMR (Preferred, Epic experience is a plus)

Knowledge/Skills/Abilities:
Communication skills consistent with the facilitation of health and wellness topics to individuals, small and large groups of community residents.
Demonstrates the ability to read, write and speak English and Spanish without an interpreter. 

Demonstrates interpersonal/cultural sensitivity and work with a diverse and often underserved population.  

Effective written and verbal communication across all levels of the organization and with vendors. ? 

Highly organized with strong time management skills, being able to perform simultaneous tasks, prioritize them effectively, and follow through quickly. 

Proficiency with EMR systems. 

Available to work Saturdays as business needs dictate.
