Job Description - Medical Case Manager


Summary: The Ryan White PART C Senior Medical Case Manager’s role is to provide medical case management services to Southwest Community Health Center's patients, including, treatment adherence, psychosocial, education, referral, client centered services that link clients with health care, and crisis intervention.  

SPECIFIC DUTIES AND RESPONSIBILITIES:  All duties are subject to accommodation in accordance with the Americans with Disabilities Act (ADA). 

  • Demonstrate professional interdisciplinary skills and provide comprehensive Medical Case Management services to clients (including treatment adherence counseling). 
  • Follow established Ryan white Medical Case Management program and policies. 
  • Assist clients in obtaining appropriate services, including medical care, and social service needs. 
  • Coordinate access to medically appropriate levels of health and support services and continuity of care 
  • Develop initial/follow-up assessment of service needs. 
  • Develop comprehensive, individualized service plan 
  • Collect and aggregate data required for The HIV Program. 
  • Assist Infectious Disease and Internal Medicine department in data collection from Southwest EMR into CAREWARE to fulfill grant requirements. 
  • Attend department meetings, medical case management meetings, conferences, monthly conference calls, and required staff meetings. 
  • Function as a collaborative member of the Ryan White Part C Program Team. 
  • Perform other duties as assigned. 

Requirements

Qualifications:  

  • Excellent writing, speaking and organizational abilities. 
  • Ability to effectively manage multiple tasks in a fast-paced environment. 
  • Must be a self-starter with the ability to work independently and as a team.  
  • Computer knowledge required. 
  • Willing and able to travel monthly out of Fairfield County. 
  • Strongly Preferred: Bilingual and/or Multilingual in English and Spanish. 

Education and/or Experience:  

  • Bachelor’s degree in human services, Psychology, Public Health or related education or three years related experience

Specific vision abilities required by this job include close vision, depth perception and the ability to adjust focus. Generally, the working conditions are good with little or no exposure to extremes in health, safety hazards and/or hazardous materials. This person must have the ability to travel as required to work with staff.
