HSNT is seeking a Medical Case Manager that is passionate about coordinating client health care, counseling, and referral services for patients and families that may have very limited options for their medical needs.

HSNT’s Medical Case Manager will become a part of a tight-knit, interdisciplinary medical team, which removes biopsychosocial barriers and supports patients by providing education and resources in a community health setting, with a patient centered medical home and value-based care focus.

Individuals that are successful in the Medical Case Manager role tend to be customer-focused and detail oriented, possess a true desire to serve a diverse population of patients and can do so without bias, and demonstrate excellent communication skillsets. It’s also helpful if candidates have knowledge of some community resources or programs and have some familiarity with various databases and EMR systems, along with Microsoft Word and Excel. A day in the life of our Medical Case Manager may look like this:

  • Assist patient, parents, and family to receive medical care through coordination with the primary care physician, nurse practitioners, emergency medical providers and other community-based service organizations.
  • Work with patients, parents, and families to assess their strengths and challenges as they address medical issues, parenting, and school related issues.
  • Meet with new families to assess socio-economic needs that may impact their responsiveness to care.
  • Perform psychological/social assessments.
  • Advocate for patients/parents/families within the organization and community to ensure access to health care services.
  • Become proficient at appropriately linking patients to care within their communities.
  • Provide clients with all family planning options available to them as allowed.
  • Adhere to the standard operating procedures and policies of Medial Case Management and the Agency.
  • Provide one-on-one client healthcare prevention education and information updates.
  • Assume a supportive role, responsible for communications and positive contributions to the team members’ efforts when working with the clients.
  • Participate in Quality Management studies.
  • Complete all required reporting in a timely manner.
  • Perform additional duties as assigned by the Director of Programs.
  • Regular in-person attendance is required as an essential function for this position.
  • Must have the ability to work efficiently under stressful conditions and or environment in a timely manner.
  • Ability to operate and drive a vehicle safely during work hours.

Requirements

Required Knowledge/Skills/Abilities:

  • Familiarity and comprehension of medical terminology.
  • Must have proven skills in working independently, self-motivated, goal oriented, strong initiative, and be a self-starter.
  • Ability to organize and prioritize.
  • Strong communication skills both verbal and written.
  • Exceptional customer service skills
  • Proficiency with data entry (including familiarity with electronic health record systems).
  • Strongly developed attention to detail.
  • Ability to adapt easily to changing environments.
  • Ability to multi-task in a busy environment.
  • Ability to communicate effectively with all levels of the organization.
  • Ability to work with people and work under pressure.
  • Ability to maintain confidentiality.
  • Ability to relate to individuals from a variety of racial, ethnic, religious, and cultural/socioeconomic backgrounds, including various sexual orientations.
  • Must be willing to work at any HSNT location, other than assigned site and be agreeable to work weekends, as responsibilities may dictate.
  • Ability to travel to all HSNT Medical Centers.
  • Bilingual skills (Spanish/English) strongly preferred

Education and Experience:

  • Education: Bachelor’s degree in social work or related field, or commensurate with experience may be considered in Lieu of education pursuant to grant requirement.
  • Licensure: Texas Board of Social Worker licensure preferred.
  • Experience providing social service support in a clinical setting.

Work Environment:

  • The activity for this position requires the employee to work in-person onsite in a clinical setting.
  • May be required to work alternative schedules.
  • Must be able to move, transport, position, and lift up to 15 pounds as needed.
  • Must be able to remain in a stationary position such as standing or sitting for up to 8 hours per day.
  • Must be able to walk, move, and/or traverse as needed.
  • Must possess the ability to communicate, detect, converse with, discern, convey, express oneself, and exchange information fluently and frequently.
  • Must possess the ability to retain information.
  • Must be able to observe, assess, recognize, identify, determine, and perceive at all times.
  • Must be able to operate a computer and other office productivity machinery at any given time.
  • Must possess a valid driver’s license.
  • Must be able to successfully complete a motor vehicle background report prior to hire date, and during employment as needed
