Rehabilitation Associates, a leading multispecialty group in New England, is seeking a BC/BE Internal Medicine, Family Medicine, or Geriatrics Fellowship trained physician to join our dynamic team of dedicated and experienced practitioners. We provide comprehensive, on-site medical care to a diverse range of patients in post-acute rehabilitation, long-term care, and assisted living centers. Our practice spans the full spectrum of post-acute settings, from acute inpatient rehabilitation to the management of complex geriatric cases in skilled nursing facilities. Responsibilities:
Collaborate with a seasoned team of Advanced Practice Providers who provide ongoing and primary management of patients
Provide care for subacute and long-term care patients at the facility, seeing approximately 15-20 patients per week, per facility.
Perform regulatory visits and oversee new patient admissions in the subacute rehabilitation unit once per week per facility.
Participate in rehospitalization, acute care transfer and quality assurance meetings to maintain and improve patient care
Complete required documentation, including patient admission and progress notes, which may be done remotely from home
Enjoy no call coverage and the convenience of having admission and discharge orders and initial summaries completed by an Advanced Practice Provider
Qualifications:
MD or DO with a valid Maine medical license
Board certified or board eligible in Internal Medicine, Family Medicine, Geriatrics, or a related field
Prior experience in subacute or long-term care settings is preferred
Strong communication and interpersonal skills
Commitment to providing high-quality patient care
Ability to work effectively in a collaborative, team-based environment
