Primary Accountability

Description of Primary Responsibilities

1. Provides overall management and direction to departmental/division staff.
   
   
   
   1. Responsible for performance management of departmental employees, including all managers, supervisors, and employees in the department, including recognition, performance evaluations, formal coaching and counseling, and making decisions or recommendations regarding necessary disciplinary actions.
   
   
   2. Responsible for recommending hiring or firing, and the advancement and promotion of managers, supervisors, and employees in the department, or any other change status of manager, supervisors, and employees in the department employees. 
   
   
   3. Demonstrates core leadership behaviors and team one approach.
   
   
   4. Demonstrates a high level of emotional intelligence.
   
   
   5. Creates a culture of accountability and excellence.
   
   
   6. Drives execution and innovation.

   11. Ensures development of department/division managers and supervisors receive instruction/training that is in compliance with training plan, including on the job training to develop department employees. Works with manager and/or supervisor to ensure necessary remediation is taken with department/division employees assigned.
   
   
   12. Assists with the development of assigned departmental/division budget(s) and monitors budget to ensure expenses do not exceed budget.
   
   
   13. Ensures regulatory compliance for assigned departments/divisions, and compliance with all workflows, policies, and procedures.
   
   
   14. Recommends workflow, policy, and procedure changes based on observations from performance metrics, outcomes, and feedback from assigned leadership team members
   
   


2. Ensures compliance with all employee-related reporting.


3. Responsible for the direct provision and supervision of professional care services.


4. Responsible for the preparation and implementation of orientation schedules for staff members.


5. Accountable for providing excellent customer service and care. 
   
   
   
   1. Consistently behaves with courtesy when interacting with patients/ family members, support staff, and Network staff.
   
   
   2. Remains aware of timely delivery of care.
   
   
   3. Ensures patients are provided educational materials and resources appropriate to the patient’s health literacy needs.
   
   


6. Accountable for patient status as measured by relevant clinical quality measures.
   
   
   
   1. Promotes and participates in daily huddles. 
   
   
   2. Incorporates the participation of ancillary health team members in managing the care of patients / families.
   
   
   3. Responsible for assisting in the assurance of clinical procedures and the maintenance of up-to-date clinical protocols.
   
   
   4. Completes chart and peer review responsibility in a timely manner.
   
   


7. Assists the patient care team with performance improvement efforts, staff education and FHCN promotion.
   
   
   
   1. Responsible for ensuring teams participation in performance evaluations and quality improvement.
   
   
   2. Where relevant, responsible for assisting in the preceptorship of mid-level practitioners.
   
   
   3. Where relevant, participate in rotation of medical, physician assistant students, and dental residents.
   
   
   4. Participates in health fairs & other community events. 
   
   
   5. Participates in Network meetings in a meaningful way.
   
   


8. Meets licensure and credentialing expectations.


9. Responsible for adhering to the Attendance and Absenteeism Policy, recognizing that regular attendance is considered an essential function of all FHCN positions.  Absenteeism is not being at work or failing to attend a paid workshop, training, or event unless the absence is protected by law. 


10. Ability to present to and work at any FHCN location, both at the beginning of a shift or during a shift, based on business need.


11. Performs other duties as assigned.

Additionally:

If assigned as a Medical Director for Outpatient Services:

1. Responsible for providing input in regards to outpatient clinical and professional programs. 
   
   
   
   1. Assists in the design, implementation and evaluation of integrated programs, including the Patient Centered Health Home.
   
   


2. Responsible for the direct provision of outpatient primary care medical services meeting all established productivity expectations.
   
   
   
   1. Will provide examination and will review appropriate laboratory, referral and imaging results to determine a diagnosis.
   
   
   2. Expected to document visits accurately utilizing appropriate templates, and record relevant data to attain a diagnosis.
   
   
   3. Accountable for attaining budgeted visits. 
   
   
   4. Responsible for meeting clinical care measures.
   
   
   5. Accountable for locking records in the 72 hour window.
   
   


3. Responsible for ensuring teams meet the primary care needs of the communities we serve and the organization meets clinical goals.
   
   
   
   1. Provide comprehensive, continuous and coordinated medical care for acute and chronic illness at all ages within the context of family unit in a community setting.
   
   
   2. Comprehensive health care including health promotion and maintenance, prevention of illness and disability and application of holistic healthcare model with focus on addressing both protective factors and risk factors affecting the health and wellbeing of patient.
   
   
   3. Management of chronic diseases and acute illness with broad range knowledge in procedural skills, coordination of care with specialists, and utilization of community resources when appropriate.
   
   
   4. Advocate access to health care with focus of structured evaluation, early intervention and health promotion. 
   
   

If assigned as the Medical Director specialty care programs and inpatient services:

1. Maintains local hospital call schedule within guidelines of larger hospital strategy. 


2. Responsible for collaborating with the operational team to provide and assess Specialty Care Services needs.
   
   
   
   1. Responsible for strategically assessing Specialty Care gaps and their impact, including financial costs, revenue, and patient care outcome objectives for established and newly adopted services. 
   
   
   2. Responsible for collaborating with the Managed Care department to strategically align efforts, optimize managed care incentives, and reduce the total cost of care.
   
   


3. Responsible for overseeing the South Valley discharge clinic operations by facilitating the transition from Inpatient to Outpatient care and actively working to reduce length of stay and readmission rates by providing comprehensive discharge services. 


4. Responsible for managing hospitalists while closely collaborating with the outpatient setting to enhance patient outcomes and incentives for shared savings.


5. Responsible for implementing inpatient strategies for OBGYN and Pediatrics across the network in collaboration with the larger team. 


6. Responsible for collaborating with local medical directors to ensure call schedules are complete and aligned with the inpatient strategy. 

If assigned as Medical Director for PACE:

1. Responsible for delivery of care and clinical outcomes. 


2. Provides medical guidance and supervision of medical services activities.


3. Provides leadership and medical expertise in the development of medical policies, procedures, and guidelines.


4. Responsible for the development of FHCN PACE clinical standards and medical practice guidelines and protocols.


5. Provides oversight of the primary care staff to include physicians and midlevel providers.


6. Functions as a primary care physician within the interdisciplinary team (IDT), including providing clinical supervision to medical care personnel such as midlevel providers.
   
   
   
   1. Performs comprehensive history and physicals on new referrals.
   
   
   2. Performs interval history and physicals on established participants, completed quarterly or as indicated.
   
   
   3. Participates as needed in the delivery of primary care.
   
   
   4. Coordinates the 24-hour care delivery.
   
   
   5. Documents participant changes appropriately in the medical records and communicates participant changes to IDT in a timely manner.
   
   
   6. Participates as a member of the IDT.  Attends staff members and takes part in participant care planning.
   
   


7. Responsible for the compliance and oversight of the Medicare Part D Drug Management Program.


8. Provides oversight of the Quality Improvement (QI) Plan.


9. Reviews all quality-of-care issues and oversees the development and implementation of quality-of-care corrective action plans.


10. Participates in the oversight, training, and education of the interdisciplinary team. 


11. Coordinates performance appraisal of the staffing under his/her oversight, including providers. 


12. Develops educational and other programs to build the skills of participating providers.


13. In conjunction with the PACE Program Director, manages all communication with the provider network.


14. Represents FHCN PACE to external agencies, professional groups, and regulatory agencies and organizations as required.


15. Demonstrates necessary skills and knowledge as outlined in position-specific competency requirements.


16. Meets Network and credentialing expectations for licensure and employment.

Description of Primary Attributes

Professional & Technical Knowledge:

Effective June 1, 2026, all individuals hired into the role must:

1. Possesses specific advanced knowledge and skills, including written and verbal communication skills, computational, computer, and technical skills, and mathematical knowledge, frequently acquired through one of the following:
   
   
   
   1. Completion of a Doctoral-level degree program with a recognized major with a minimum cumulative GPA of 2.5; or
   
   
   2. Successful attainment and maintenance of a valid, active professional license required for the role, issued by the appropriate governing or regulatory body.                                                                                               
   
   


2. Six years of leadership experience or seven years of progressively greater responsibility or significant contributions to projects and initiatives that demonstrate leadership skills.


3. Performance of the duties and responsibilities of the job requires extensive formal training in medical studies, including an understanding of the application of the theory and practices of medicine.

If assigned to PACE:

1. Must have experience working in a managed care environment and working with peers and other health providers to resolve utilization, quality management, performance improvement, pharmacy and therapeutics, peer review, credentialing, and physician leadership issues. 


2. Either have one year of experience working with a frail or elderly population or, in the absence of such experience, to receive appropriate training from FHCN PACE on working with a frail or elderly population upon hiring.
   
   
   
   1. Able to work within an interdisciplinary team setting.
   
   


3. Have a minimum credit score of 650.   

Technical Skills:

1. Ability to prepare more complex documents in Microsoft Word, including creating tables, charts, graphs and other elements.


2. Ability to use Microsoft Excel to analyze data, including the use of formulas, functions, lookup tables and other standard spreadsheet elements.


3. Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions and other elements.

Licenses & Certifications: 

1. Licensed to practice medicine in the State of CA.  If assigned to PACE, must have completed study in Family Medicine or Internal Medicine, with advanced certification in geriatrics preferred.


2. DEA registration and the ability to obtain and maintain staff privileges at FHCN PACE contracted agencies. 

Communications Skills:

1. Job duties require the employee to effectively communicate, verbally and in writing, their opinions and extrapolations of information they collect and synthesize/analyze.


2. Responsible for the resolution of conflicts that may arise because of disagreements between employees, between employees and customers/clients, or with the public, other legal entities or governmental authorities.


3. Compiles, analyzes and prepares information in an effective written form, including correspondence, reports, articles or other documentation.


4. Effectively conveys technical information to non-technical audiences.

While performing the duties of this position, the employee is regularly required to sit and use repetitive hand movement to type and grasp. The employee is frequently required to stand or walk; and occasionally lift and/or move up to 20 pounds. 

Min Salary Range: $309,322.82

Max Salary Range: $494,916.51
