Job Description - Medical Lead

Mains’l Services is seeking a Medical Lead in our Dayton, MN location. As the medical lead you will coordinate, document, and implement medical supports to help the individuals we serve access proper medical care and live healthy, meaningful lives. The medical lead will help coach and mentor Mains’l staff by leading by example, using Mains’l’s mission, vision and values.

What You’ll Do:

Provide Person Centered Supports tailored to the individual needs (including Activities of Daily Living, behavioral supports, coaching through different situations, participating in home and community activities)
Coaching and collaboration with the team
Documenting care accurately to ensure ongoing support
Maintain a safe and structured environment
Assist with Coaching and Training of Direct Support Professionals
Support individuals in learning new skills based on their interests and preferences
Coordinate, schedule and accompany individuals to medical appointments
Review medical referrals, completing follow-ups and communicate medical updates
Create weekly menu and grocery shop based on needs

Location: Dayton, MN
Work Structure: 10am-6pm, including every other weekend
Wage: $23.00/Hour starting
What You Need:

1+ Years of CNA or Related Experience
1 year of Leadership Experience (preferred)
Must be at least 18 years old
Valid MN driver’s license
Reliable transportation with valid insurance
Ability to pass a DHS Background Check
Knowledge and understanding of Person-Centered Practices and Positive Supports
Able to think outside of the box
