As part of the LIFE's Senior Services PACE (Program of All-Inclusive Care for the Elderly) team, we perform a vital service by providing primary care for Tulsa's aging population. Our amazing team of professional’s provide services and oversight to our most vulnerable population by helping participants stay in their homes for as long as safely possible. We are looking for someone who is friendly, warm and professional with a compassionate heart to join our team!  

The Medical Records Clerk will be supporting our PACE Clinic.  In this full-time role, you will be responsible for scanning documents, updating the electronic health record, requesting records from specialists and hospitals, uploading documents into the electronic medical record and responding to release of information (ROI) requests. As part of this role, you will ensure compliance with state and federal regulations, including HIPAA guidelines.

The ideal candidate will be able to work effectively in a team environment, able to multi task, detail-orientated, analytical skills and have the ability to maintain confidentiality and communicate effectively.  

 RESPONSIBILITIES:

· Maintains a single comprehensive medical record for each participant enrolled in the LIFE PACE program. Participant medical records should be complete, accurate, easily retrievable, systematically organized, and available to staff and participants when requested.

· Provides for the prompt transfer of copies of appropriate medical record information between treatment facilities to ensure continuity of care whenever a participant is temporarily or permanently transferred to another facility. Uploads physician, specialist and other provider reports and hospital/ER summaries into the EHR system. Scans other reports and documents into the EHR system. Correctly files all documentation into participant’s chart, in a timely manner.

· Ensures data and information is entered into the EHR in an efficient, accurate, and timely manner, and that all incoming Electronic Health Records (EHR) documents are accurate and complete (for name, spelling, signatures, etc.).

· Monitors all active participant records to ensure accuracy and compliance. Maintains knowledge of EHR software as it relates to the Medical Record. 

· Documents clearly in the EHR when appointments are canceled and rescheduled.

· Assists with scheduling outside appointments and scheduling transportation for these appointments. Notifies participants of scheduled appointments and sends reminders by mail, phone, email, and Voice-friend (or equivalent).

· Brings participant requests for additional services to an IDT member as soon as possible upon request.

· Actively seeks to obtain medical records before and after hospitalizations, outside appointments, SNF stays, pre-enrollment visits, etc. 

· Coordinates with other team members and disciplines to maintain an accurate and complete medical record; provides training as requested.

· Attends and participates in morning meetings, staff meetings, in-services and projects and committees as assigned. Participates in special projects and performs additional duties as required or assigned.

· Adheres to and supports the organization’s policies, practices and procedures.

Requirements

EDUCATION & CERTIFICATIONS:

· High School diploma or equivalent required. 

· A minimum of one-year experience working in a busy medical office using an EMR system. 

· A minimum of one-year experience working with the elderly population preferred. 

· High volume medical records experience preferred.
