Medical Social worker for Hospice

Description of the role:

University Home Care is seeking a passionate and dedicated Medical Social Worker to join our team in providing hospice care in Livonia, MI. The ideal candidate will have experience working with terminally ill patients and their families, providing emotional support and guidance during difficult times.

Responsibilities:

• Provide counseling and emotional support to hospice patients and their families


• Assess the psychosocial needs of patients and create care plans


• Collaborate with other healthcare professionals to ensure comprehensive care

Requirements:

• Master's degree in social work


• Limited license master social worker/Full license 


• Licensed as a social worker in the state of Michigan


• Experience working with hospice or palliative care patients
