Medical Social Worker (MSW) - Home Health.

Location: O’ahu

Always Best Care Senior Services - O’ahu is hiring a Medical Social Worker to join our home health team. You will help patients and families work through the emotional, financial, and practical challenges that come with illness and recovery, right in their own homes.

What you will do

• Assess patients’ psychosocial and emotional needs, family dynamics, home environment, and financial concerns


• Provide counseling, emotional support, crisis intervention, advocacy, and education


• Connect patients and caregivers with community resources, including financial, housing, transportation, legal, and long-term care services


• Support discharge planning and transitions of care


• Coordinate with physicians, nurses, therapists, and the rest of the care team, and take part in case conferences


• Keep accurate, timely assessments, visit notes, and progress documentation


• Take part in quality improvement (QAPI) activities

Qualifications

• Master’s degree in social work


• Current Hawaii LSW license that meets home health medical social services requirements


• At least one year of experience in healthcare, home health, or social services preferred


• Knowledge of community resources and social service programs


• Strong assessment, counseling, communication, documentation, advocacy, and organizational skills


• Valid driver’s license, reliable transportation, and automobile insurance when required by agency policy
