The Davis Community requires that all current and new employees, including contract staff, receive an annual influenza vaccination and TB skin test unless a reasonable or disability accommodation is granted.

BASIC PURPOSE:

The Special Care Coordinator is responsible for the administration of medication, coordination of services to residents and provides direct supervision to PCAs and CNAs on the Special Care unit under the direction of the Resident Care Director. All responsibilities will be conducted in a manner that is consistent with the assisted living philosophy. Provides training related to dementia and special needs as governed by state and federal guidelines.

MAJOR WORK ACTIVITIES:

1. Oversees care management of residents in the Special Care unit ensuring appropriate information is communicated to the Resident Care Director and family members.

2. Provides assistance or guidance to residents for any/all appropriate activities.

3. Monitors and reports any change in physical or mental health of the residents to the physician and any other staff as appropriate.

4. Participates in and assists with resident-centered activity program.

5. Assesses quality of care issues on Special Care unit on a continuous basis through conducting rounds monitoring resident care, documentation, and investigation of abuse/neglect, grievances and family concerns.

6. Oversees daily unit staff assignments and shift scheduling requirements, as needed ensuring assigned tasks are completed.

7. Train, supervise, motivate and evaluate performance of staff on Special Care Unit (30,60,90,annual) to ensure the provision of high quality services are delivered to residents.

8. Responsible for new staff orientation, develop and implement in-service training programs in accordance with regulations regarding dementia.

9. Performs monthly audits of Med Techs and record in Inspection Log on Special Care unit.

10. Provides objective information to Resident Care Director concerning care and condition of each special care resident. Participates in Special Care residents’ care plan meetings and periodically revises special care assessments.

11. May assume responsibility of filling in for any position as needed.

12. Assures all shift documentation and reports are complete and conveyed to the on-coming shift including status reports and acute charting.

13. Initiate and conduct disciplinary action process with employee as needed.

14. Supervises personnel in other departments as needed with regard to the delivery of services to ensure a safe, clean and pleasant environment in the Special Care unit.

15. Works towards maintaining each resident’s self-respect, personal dignity, and personal safety.

16. Performs other nursing services and responsibilities as assigned by the Resident Care Director.

17. Performs medication administration to residents and documentation including ordering, returning medication and maintaining MARs on Special Care unit.

18. Obtain Lab specimen per physician order and monitor results on Special Care unit, alerting M.D. as needed.

19. Responsible for all infection control duties, including immunization updates and chest X-Rays on Special Care unit.

20. Gets nursing supply list for Special Care unit to Medical Records Assistant to order as needed.

21. Acts as Resident Care Director’s designee in his/her absence.


Requirements

KNOWLEDGE, SKILLS WORKING CONDITIONS:

· Works in a well-lighted office area and/or neighborhood

· Low exposure to infectious diseases

· May stand, stoop, bend and walk intermittently during the workday

· Must be able to lift at least 30-40 pounds

· Good planning, initiation, delivery and evaluation skills with regard to nursing care

· Knowledge of documentation requirements related to timeliness, accuracy and completeness

· Working knowledge of State regulations as they apply to the assisted living setting

· Exposure to peculiarities in behavior and reactions of the aged, including combative residents

MINIMUM QUALIFICATIONS:

Education: Associates Degree in Nursing from an accredited school of nursing

Licensure/Certificate: Current North Carolina licensure as a Licensed Practical Nurse

Experience: Must be 21 years or older. Must have at least 2 years of previous work experience in geriatric nursing.

An equivalent combination of education and experience may be considered.
